| ObjectiveCollecting,sorting and analyzing the first 65 young patients with colorectal cancer diagnosed in our hospital and undergoing surgical treatment clinical data such as symptoms,onset time,auxiliary examinations,postoperative pathological data and other information,to explore the clinicopathological characteristics of young patients with colorectal cancer and their correlation with prognosis,in order to understand the characteristics and rules of young colorectal cancerMethodsThe data of 65 young patients with colorectal cancer treated in our hospital from January 2005 to December 2014 were retrospectively analyzed,including the first symptoms,release time,auxiliary examinations,postoperative pathology and follow-up data.Results1.In this study,there were 31 males and 34 females,and the ratio of male to female was 1:1.1;The youngest was 20 years old and the oldest was 45 years old,with an average age of 33.86±50 years old.2.In this study,among the first symptoms of young patients with colorectal cancer,20 patients had hematochezia as the main first symptom,13 patients had stool changes and abdominal pain as the first symptom,11 patients had abdominal distension as the first symptom,and 4 patients had lesions found during routine physical examination.In addition,2 patients presented with pale complexion and fatigue after activity,1 patient presented with tenesmus,and 1 patient presented with abdominal mass.The shortest time of diagnosis was 1 day and the longest was 2 years,with an average of(4.59 1.67)years ±20)months.3.Among the 65 young patients with colorectal cancer,7 were underweight,45 were normal weight and 13 were overweight;There were 39 patients with normal CEA and 26 patients with elevated CEA;There were 25 patients with anemia before operation and 40 patients without anemia.4.Among the 65 young patients with colorectal cancer,28 cases were located in the rectum,22 cases in the left colon and 15 cases in the right colon;The degree of tumor differentiation was mainly low and middle differentiation with poor differentiation;There were 50 cases of adenocarcinoma,11 cases of mucinous adenocarcinoma and 1 case of signet ring cell carcinoma;There were 14 cases of vascular tumor thrombus and 19 cases of nerve invasion;In TNM staging,there were 22 patients with stage Ⅰ and Ⅱ,and 43 patients with stage Ⅲ and Ⅳ.5.As of December 2019,60 patients had been followed up,with a follow-up rate of 92.3%.17 patients were still alive at the deadline of follow-up.6.Univariate analysis showed that the degree of tumor differentiation,T stage,N stage,TNM stage,vascular tumor thrombus,nerve invasion,surgical methods were correlated with the prognosis of patients(P<0.05).Multivariate analysis showed that T stage,TNM stage and nerve invasion were independent prognostic factors of young patients with colorectal cancer(P<0.05).ConclusionThe early symptoms of colorectal cancer in young adults are usually atypical,and most of them are sporadic cases.Most patients have poorly differentiated tumors,late stages,and high malignancies when they are diagnosed,leading to a poor prognosis,causing a huge economic and psychological burden to patients and their families.Therefore,clinicians should be vigilant,especially for patients with a family history of polyps or gastrointestinal malignancies,they should be advised to perfect the relevant examinations so as not to miss the opportunity for diagnosis and treatment. |
