| Objective:Today,tuberculosis remains a serious threat to human health.Laboratory diagnosis of tuberculosis remains difficult,highlighted by the lack of a highly sensitive and specific diagnostic method.The purpose of this study was to evaluate the clinical value of simultaneous RNA amplification test for Mycobacterium tuberculosis(SAT-TB)combined with other diagnostic methods in the diagnosis and treatment of tuberculosis.Methods:A total of 147 patients diagnosed with tuberculosis were collected from Anhui Chest Hospital between January 2017 and January 2019.The sputum and body fluid(including pleural effusion,ascites,cerebrospinal fluid,pus,bronchial and alveolar lavage fluid,etc.)were detected by real-time quantitative thermostatic nucleic acid amplification test(SAT-TB),acid-fast bacilli smear,tuberculosis antibody test(TB-Ab),enzyme linked immunodot test(T-SPOT)and fluorescence quantitative polymerase chain reaction(PCR).(hereinafter referred to as"body fluid"),blood samples for testing,Comprehensive evaluation of real-time quantitative thermostatic nucleic acid amplification test(SAT-TB),acid-fast bacilli smear,enzyme-linked immunodot test(T-SPOT),tuberculosis antibody test(TB-AB),fluorescent quantitative polymerase chain reaction(PCR),SAT-TB combined with T-SPOT,SAT-TB combined with T-SPOT,TB-AB,SAT-Application value of TB combined PCR in the diagnosis of tuberculosis.Results:Compared with acid-fast bacilli smear of sputum and body fluid,the sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB in the diagnosis of tuberculosis were 47.62%、79.37%、75.47%、53.19%and 61.22%,respectively,and the difference was statistically significant(?~2=11.37,P<0.05).Compared with TB-Ab test,the sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB were 28.44%、65.79%、70.45%、24.27%and 38.10%,respectively,and the difference was not statistically significant(?~2=0.45,P>0.05).Compared with T-SPOT test,the diagnostic sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB were 25.00%、68.57%、77.79%、22.22%and 35.37%,respectively,and the difference was not statistically significant(?~2=0.57,P>0.05).Compared with PCR test,the diagnostic sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB were 52.94%、80.65%、78.94%、55.56%and61.22%,respectively,and the difference was statistically significant(?~2=17.04,P<0.05).Compared with acid-fast bacilli smear of sputum and body fluid,the sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB combined with TB-Ab were 42.06%、80.95%、92.98%、18.89%and 47.62%,respectively,and the difference was statistically significant(?~2=4.02,P<0.05).Compared with acid-fast bacilli smear of sputum and body fluid,The sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB combined with TB-Ab and T-SPOT were 41.94%、82.61%、92.86%、20.88%and 48.30%,respectively,and the difference was statistically significant(?~2=4.96,P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and coincidence rate of SAT-TB combined PCR were73.24%,81.58%,78.79%,76.54%and 77.56%,respectively,with statistically significant differences(?~2=44.59,P<0.05).Conclusion:1.By comprehensive comparison of the application of SAT-TB test,acid-fast bacilli smear test,TB-AB test,T-SPOT test and PCR test in the diagnosis of tuberculosis,it was found that the diagnostic consistency of SAT-TB test,PCR,acid-fast bacilli smear test was relatively good,and the diagnostic consistency of SAT-TB test was poor with other tests.2.Comparing the three diagnostic methods of SAT-TB test,SAT-TB combined with TB-AB test and SAT-TB combined with TB-AB test and T-SPOT test,it was found that the sensitivity,specificity and coincidence rate of the diagnostic results of SAT-TB combined PCR test were the best.That is to say,SAT-TB combined PCR test can effectively and accurately diagnose tuberculosis.Compared with single diagnostic methods such as TB-AB,T-SPOT and PCR,combined diagnostic methods have higher specificity. |
