Background The assessment of jaw bone is widely used in the diagnosis and treatment of oral diseases.Bone assessment included bone density and trabecular structural characteristics.Bone mineral density refers to the mineral content in bone tissue per unit volume,which is directly related to bone strength.Clinically,the grayscale value measured by cone beam CT is commonly used to represent bone mineral density,but the grayscale value cannot reflect the difference of bone trabecular microstructure.Bone trabecula is the part of dense bone extending to cancellous bone,showing a porous network structure.Hematopoietic cells and connective tissue form bone marrow to fill the gap between them.The three-dimensional structure of bone trabecula directly affects bone healing and bone strength.The evaluation of trabecular structure has been widely used in the analysis of bone function and strength,grafting healing,bone diseases(such as osteoporosis),bone mass before grafting,and osteointegration after grafting.At present,micro CT is commonly used to evaluate bone trabecular structure,but with high radiation dose,micro CT is limited to evaluating small bone samples and cannot scan clinical patients.Conical CT has been widely used in oral and maxillofacial imaging,which has good spatial resolution,gray density range,contrast,and good image to noise ratio.In recent years,scholars have used conical CT and microscopic CT to conduct in vitro scanning of human jaw samples to quantitatively measure the bone morphology.The results show that the three-dimensional structural parameters measured by the two are correlated,indicating that CBCT has the possibility of practical application in evaluating the microstructure of bone trabeculae in the jaw.To explore the feasibility of measuring trabecular porosity with CBCT auxiliary tool developed in cooperation with software company as an alternative parameter for evaluating trabecular structure.Methods A total of 231 tooth loss sites in 200 patients with preoperative CBCT were selected from Anhui Provincial Stomatological Hospital.The trabecular porosity was measured by CBCT auxiliary tools,and the structural parameters(BV/TV,Tb.Th,Tb.N,Tb.SP)by CTAn software,SPSS22.0 software and Pearson correlation analysis,analysis of variance and t test were used for statistical analysis.Results(1)The porosity of bone trabecular was negatively correlated with BV/TV(r=-0.761,P<0.05);and positively correlated with Tb.Sp(r=0.856,P<0.05).(2)Average trabecular porosity in the missing area: maxillary posterior region(4.1693±6.0950)% >mandibular posterior region(1.8071±2.2037)% >maxillary anterior region(1.4079±2.0512)%> mandibular anterior region(1.3309±2.4318)%,the difference of bone trabecular porosity between the maxillary posterior area and other areas was statistically significant(P < 0.05).(3)The average trabecular porosity of different age groups was significantly different.The average trabecular porosity of 30-49 age group was smaller than that of 18-29 and 50-70 age groups(P <0.05).(4)There was no significant relationship between trabecular porosity and bone mineral density(P<0.05),the mean trabecular bone porosity: bone type Ⅳ(1.9359±2.1855)% > bone type Ⅱ / Ⅲ(1.7030±4.6605)% < bone typeⅠ(1.5307 ±4.0950)%,there was no significant difference between groups(P<0.05).Conclusion In the treatment of oral diseases,CBCT can be used to measure the trabecular porosity and evaluate the changes in the structure of alveolar bone trabecular.
