Objective:Objective To observe the clinical and imaging effects of stainless steel prefabricated crown in repairing young first permanent molars with large area defect in children,and its influence on periodontal tissue and oral health quality.Methods:From March 2019 to March 2020,24 cases of children with first permanent molars who came to the Stomatological Hospital of Shanxi Medical University were collected.The teeth were all large-area defects.The self oral control experiment was carried out,so each person took two maxillary or mandibular symmetrical homonymous teeth as the research object,a total of 48 teeth.The experimental group(n = 24)was the first permanent molar with large area defect and was restored with stainless steel prefabricated crown(PMC),while the control group(n = 24)was the healthy homonymous tooth.The restoration effect of the experimental group was observed at different time points after 3months,6 months and 12 months(including tooth integrity,restoration condition,marginal tightness,secondary caries,etc.),and the periodontal health status of the experimental group and the control group at different time points before restoration,3 months,6 months and 12 months after restoration [including gingival probing bleeding(BI),gingival probing bleeding(BI),gingival probing bleeding(BI)and periodontal health status of the experimental group and the control group were observed Plaque index(PLI),the distance from alveolar crest to enamel cementum boundary(CEJ)and the score of pediatric oral health impact scale(cpqii-14)before,3 months,6 months and 12 months after restoration were measured.The incidence of complications(including secondary caries,fracture of filling body,crown abrasion,tooth fracture and crown fall off)in the experimental group was also counted.Results:(1)Basic information24 cases were observed,including 14 males(58.3%).The average age was 10.0±1.3years(range,6-12 years).The age of male children was 10.0±0.68 years old,and that of female children was 10.1±1.9 years old.The results showed that there was no significant difference between boys and girls(P > 0.05).The results showed that there was no significant difference between the teeth position and the upper and lower jaw of the first permanent molar in each quadrant(P > 0.05).(2)Repair effect In the experimental group,there was no significant difference in the restoration effect(tooth integrity,restoration condition,marginal tightness,secondary caries,etc.)after 3months,6 months and 12 months(P > 0.05).(3)Periodontal health PLI :Compared with the control group,the PLI of the experimental group before repair(3.21±0.83)was significantly higher than that of the control group(1.13±0.61)(P< 0.05).Within group comparison,the time points before repair in the experimental group(3.21±0.83)were significantly higher than the three time points after repair(0.96±0.69,0.88±0.68,0.88±0.68)(P < 0.05),and there was no significant difference in the results of each time point before and after repair in the control group(P > 0.05).BI:Compared with the control group,the time point before repair in the experimental group(2.04±1.08)was significantly higher than that in the control group(0.96±0.36),and there was no significant difference between the two groups at 3 months,6 months and12 months after repair(P > 0.05);the time point before repair in the experimental group(2.04±1.08)was significantly higher than that in the control group(0.92±0.28,0.83±0.48,0.79±0.05)(P < 0.05).There was no significant difference in the results of the three measurement nodes before and after repair in the control group(P > 0.05).Imaging: The distance between CEJ and alveolar crest in experimental group and control group was compared:(P > 0.05).There was no significant difference in the results between the three time points before and after operation(P > 0.05).(4)Complications In the 12-month observation period,there was 1 case of crown shedding in 24 patients,and no other complications occurred.The total incidence of complications was 4.2%.(5)Oral health related quality of life in children Comparison before and after restoration: at 3 months,6 months and 12 months after restoration,the scores of oral function limitation,oral symptoms,social emotional health and school environmental impact of the two groups decreased,and the difference was statistically significant(P < 0.05).Conclusion:A short-term study on the repair of large area defect of young first permanent molars with stainless steel prefabricated crown showed that:(1)Good clinical repair effect(2)There was no adverse effect on the periodontal tissue health of the children.(3)The incidence of complications was low.(4)It may improve the quality of life of children.
