Clinical Observation On The Treatment Of Gout Of Damp Heat Accumulation Type By Danggui Niantong Decoction

Objective:Danggui Niantong Decoction was used to treat gout of damp heat accumulation type in order to observe the clinical efficacy and safety,and to investigate the efficacy of lowering uric acid with Western medicine in the acute phase of gout,so as to provide new treatment plan and ideas for clinical work.Methods:From January 2019 to December 2020,60 patients with gout of dampness heat accumulation type were enrolled in the efficacy observation.They were randomly divided into conventional group,treatment group A and treatment group B,20 cases in each group.Three groups were given low purine diet and other lifestyle adjustment,the conventional group was given diclofenac sodium treatment,the treatment group A was given febuxostat on the basis of the conventional group,on the basis of the treatment group A,the treatment group B was given Danggui Niantong Decoction,the course of treatment was one week,three groups of the information were recorded before and after treatment,such as the inflammatory markers,serum uric acid,joint pain scores,TCM syndrome scores and safety indicators,and statistical analysis of data to evaluate the clinical efficacy and safety.Results:1.General informationThere were no significant differences in gender,age,course of gout,comorbid diseases and location of gout attack among the three groups before treatment(P > 0.05).2.Therapeutic Indicators(1)Comparison of the inflammatory marker(ESR):Before and after treatment,the differences in the three groups were significant(P< 0.05);After treatment,the difference about ESR between the conventional group and the treatment group A was no significant difference(P > 0.05),but the differences among the treatment group B and the conventional group or the treatment group A were significant(P < 0.05).(2)Comparison of the inflammatory marker(hs-CRP): The differences in the three groups before and after treatment was statistically significant(P < 0.01);After treatment,the difference about the result of hs-CRP between the conventional group and the treatment group A was no significant(P > 0.05),but the difference among the treatment group B and the conventional group or the treatment group A were significant(P <0.05).(3)Comparison of serum uric acid: The differences about the result of SUA in the three groups before and after treatment were significant(P < 0.05),and the differences in the treatment group A and the treatment group B was statistically significant(P <0.01);After treatment,the difference about the result of SUA between the conventional group and the treatment group A was statistically significant(P < 0.05),there was statistically significant the difference between the conventional group and the treatment group B(P < 0.01),and the difference among the treatment group A and the treatment group B was significant(P < 0.05).(4)Comparison of joint pain scores: Before and after treatment,the differences in the three groups were statistically significant(P < 0.01);the difference about the result of joint pain scores after treatment was no significant between the conventional group and the treatment group A(P > 0.05),but the differences among the treatment group B and the conventional group or the treatment group A were significant(P < 0.05).(5)Comparison of TCM syndrome scores: Before and after treatment,the differences in the three groups were statistically significant(P < 0.01);The difference about the result of TCM syndrome scores after treatment was no significant between the conventional group and the treatment group A(P > 0.05),but the differences between the treatment group B and the conventional group or the treatment group A were significant(P < 0.05).(6)Comparison of efficacy: The differences among the three groups after treatment were significant(P < 0.05).The total effective rate was 85% in the conventional group,the treatment group A was 85%,and 95% in treatment group B.3.Safety indicatorsThe difference about the result of safety indicators,such as ALT,AST,Cr and WBC,there were no significant between the three groups(P > 0.05).Conclusion:(1)Danggui Niantong decoction combined with medicine in the treatment of gout with damp heat accumulation type can further reduce joint pain scores,TCM syndrome scores,the inflammatory markers and serum uric acid,and that is high safety.Its effect is better than that of Western medicine alone.It has clinical application value,and is worthy of clinical promotion.(2)Uric acid lowering therapy of patients with Western medicine in gout acute phase can reduce the clinical symptoms,the effect of reducing uric acid is better than traditional western medicine,and it will not increase the joint pain and inflammatory index.
