Clinical Observation Of Wuwei Wentong Chubi Capsule Combined With Traditional Chinese Medicine Fumigation In The Treatment Of Ankylosing Spondylitis Of Du Han Type And Its Effect On The Levels Of IL-6,IL-23 And IL-34

1 Research backgroundAnkylosing spondylitis(AS)is a chronic immune disease.The common symptom is stiffness and pain in the lower back,and spinal stiffness and deformity can occur with increased inflammation of the spine.The most common occurrence of this disease is young and middle-aged men under 40 years of age.The etiology and pathogenesis of the disease are complex and the onset is hidden.Symptomatic treatment for the disease can effectively control inflammation and prevent bone destruction to a certain extent,but it cannot achieve the desired effect.Traditional Chinese medicine has great potential advantages and distinctive characteristics in the prevention and treatment of AS.In recent years,many clinical observation studies have begun to focus on the performance of syndromes and laboratory indicators,and analyze the distribution of indicators for patients with different syndromes,all of which provide TCM syndrome differentiation.Based on objective evidence,the remarkable effect of traditional Chinese medicine in delaying the progression of AS patients and improving clinical symptoms has also been verified in a large number of literatures.2.Research purposeObserve the improvement of Wuwei Wentong Chubi Capsules combined with traditional Chinese medicine fumigation in the treatment of Duhan type ankylosing spondylitis,and discuss Wuwei Wentong Chubi Capsules and traditional Chinese medicine fumigation for the treatment of Duhan type ankylosing spondylitis.The mechanism of TCM further verifies the efficacy of Chinese medicine in the treatment of ankylosing spondylitis,and promotes the development of clinical and scientific research.3 Research methodsAccording to the diagnostic criteria of disease and TCM syndrome,60 patients with as were selected from the outpatient and inpatients of rheumatology department of the First Affiliated Hospital of Anhui University of traditional Chinese medicine.They were randomly divided into treatment group(n = 30)and control group(n = 30).The control group was given sulfasalazine(SASP)orally,twice a day,1.0g each time;The treatment group was given Wuwei Wentong Chubi capsule orally,three times a day,three capsules each time,and combined with traditional Chinese medicine fumigation treatment,once a day,20 minutes each time,five times a week.The treatment time was12 weeks.After treatment,the TCM syndrome score,TCM syndrome efficacy,VAS score,BASDAI score,BASFI score and the levels of ESR,hs CRP,IL-6,IL-23 and IL-34 in serum were compared between the two groups.4 Research resultsThere was no significant difference in gender,age,course of disease between the two groups(P > 0.05).(1)Single TCM syndrome: after treatment,the scores of lumbar pain,limited activity and morning stiffness in the control group were lower than those before treatment(P < 0.05),while the scores of chills,four terminal tepid,joint cold pain,limb sleepiness and long urine in the control group were not statistically significant(P > 0.05);In the treatment group,the scores of lumbar pain,limited activity,morning stiffness,chilly,four terminal tepid,joint cold pain,limb sleepiness,clear and long urine were lower than those before treatment(P < 0.05),and the improvement of the above symptoms in the treatment group was better than that in the control group(P< 0.05).(2)TCM syndrome score: compared with before treatment,the TCM syndrome score of the two groups decreased after treatment(P < 0.05),compared with the control group,the TCM syndrome score of the treatment group was better after treatment(P <0.05).(3)TCM syndrome efficacy: the effective rate of the treatment group(90%)was higher than that of the control group(80%),with statistical significance(P < 0.05).(4)VAS score,BASDAI score,BASFI score: compared with before treatment,the three scores of the two groups were decreased after treatment,the difference was statistically significant(P < 0.05),and the improvement of the treatment group was more obvious than that of the control group(P < 0.05).(5)Blood indexes: after treatment,the levels of ESR,hs CRP,IL-6,IL-23 and IL-34 in the two groups were improved compared with those before treatment(P < 0.05),and the levels of ESR,hs-CRP and IL-34 in the treatment group decreased more significantly than those in the control group(P < 0.05).(6)Safety indicators: after 12 weeks of treatment,the safety indicators of the two groups were not significantly abnormal.5 Research conclusionsThe clinical application of Wuwei Wentong Chubi Capsules combined with traditional Chinese medicine fumigation treatment can better alleviate the clinical symptoms of patients with cold-regulating AS,reduce inflammatory reactions,reduce ESR,hs-CRP,and IL-34 levels,and effectively improve the patient’s functional index,Improve the quality of life,and have good safety.
