Meta-analysis And Systematic Review Of The Clinical Efficacy Of Bushen Zhuanggu Decoction In The Treatment Of Osteoporosis

Objective: Based on the theory of evidence-based medicine,the meta-analysis method is used to systematically evaluate the clinical effectiveness and safety of Bushen Zhuanggu Decoction in the treatment of primary osteoporosis,which provides a better theoretical basis for the clinical application of Bushen Zhuanggu Decoction.Method: The quantitative and qualitative data of this systematic review are all derived from manual retrieval of major electronic databases.The retrieval method is based on the PICOS principle of the Cochrane Library.The retrieval scope is all documents from the major Chinese and foreign medical literature databases from the establishment of the database to January 2021.The data time span of the Chinese database and the extracted database documents include China Knowledge Network(CNKI 1997-2019),VIP.com(VIP 2000-2018),Wanfang data(2000-2020),China Biomedical Sciences Literature database(CBM,1997-2020).The English document database and the data time span of the extracted database documents include Pub Med(2014-2014),Embase(2003-2005),Web of science(2007-2015),The Cochrane library.Retrieving all the clinical randomized controlled trials of Bushen Zhuanggu Decoction in the treatment of primary osteoporosis in the above 8 databases,statistically including the general characteristics of the literature and evaluating its quality.Use Revman Manager5.1 software for data statistics and analysis.The extracted data include effective rate, bone density level(BMD),visual analog score(VAS score),serum estradiol level(E2),TCM syndrome score and Security,etc.Outcomes:(1)Compared with the control group,the experimental group containing Bushen Zhuanggu Decoction has a higher clinical effective rate in the treatment of primary osteoporosis [OR=5.90,95%CI(4.32,8.06)Z=11.13 P<0.01].(2)High bone density [OR=1.49,95%CI(0.80,2.18)Z=4.22 P<0.01].(3)VAS score is low [OR=-5.66,95%CI(-8.18,-3.14)Z=4.40 P<0.001].(4)The level of estrogen is high [OR=17.84,95%CI(7.75,27.92)Z=3.47 P<0.0005].(5)Low TCM syndrome score [OR=-3.01,95%CI(-3.61,-2.60)Z=12.00 P<0.00001]。(6)High safety [RR=0.62,95%CI(0.26,1.44)Z=1.12 P=0.26].Conclusion:(1)Compared with the pure western medicine group,Bushen Zhuanggu Decoction can significantly improve the clinical effectiveness.(2)Significantly increase the bone density of patients with osteoporosis.(3)It can significantly increase the estrogen level of patients with osteoporosis.(4)It can significantly reduce the VAS score of patients with osteoporosis.(5)Compared with the simple western medicine group,it can reduce the TCM syndrome score and has high safety,but the results of the two types of indicators are very heterogeneous,and a large sample is needed for further confirmation.
