Current Situation Of Prevention-treatment Integration Under The Family Doctor System In Chongqing J Districtand Optimization Suggestions

The integration of prevention and treatment under thefamily doctor system is asignificant part of the reform of the medical and health system and an effective means to effectively solve the problem of ―difficult and expensive medical treatment‖.Effective implementation of the integration of prevention and treatmentmodel under the familydoctorsystem willimprove the ability of family doctors,improve service quality,enhance job performance and reflect personal value,so as to better meet the high-quality and diversified needs of the masses and enhance the sense of gainof residents.As early as 2009,the Ministry of Health,the Ministry of Finance,the Ministry of Population and Family Planning and other departments issued the―Opinions on Promoting the Gradual Equalization of Basic Public Health Services‖and the ―National Standards for Basic Public Health Services(2009 Edition)‖ and other documents,clarifying that township health centers,village clinics,community health service centers(stations)and other urban and rural primary medical and health institutions are the main service providers.In the context of the implementation of basic public health services in primary medical institutions for many years and the disconnection of prevention and treatment,in March 2018,the National Health Commission issued the ―Notice on Doing a Good Job in Contracting Services for Family Doctors in 2018‖,requiring that ―Family doctor teams should meet the service needs of contracted residents to provide integrated and continuous medical and health services with integration of prevention and treatment.‖ Simultaneously,the Chongqing Municipal Health and Family Planning Commissionalso formulated the ―Notice on Carrying Out the Pilot Work of Establishing a Development Mechanism for the Integration of Preventionand Treatment in Primary Medical and Health Institutions‖.It can be seen from this that the state and Chongqing Municipality attach great importance to the provision of integration of prevention and treatment services by grassroots family doctors,and the series of documents formulated have pointed out the direction for the development of integration of prevention and treatment and the solution to the problem of disconnection of prevention and treatmentBased on the review of existing literature,this article sorts out the development process of the integration of prevention and treatment in the country and Chongqing,and explainsa new service model of integration of prevention and treatment under the family doctor system.Aiming at the current status of the implementation of the integration of prevention and treatment in J district,through field surveys,anonymous questionnaire surveys and interviews,this article finds out the existing problems and deficiencies.From a macro point of view,there are phenomena such as uncompleted working mechanisms,insufficient policy support and insufficient talent team.From a micro point of view,there are problems like insufficient optimization of service processes,unchanged concept of doctors,low level of grassroots services,uncompleted information integration,and insufficient participation of residents.Meanwhile,in view of these problems,carefully analyzing the reasons,the following suggestions are proposed: to expand the channels of talent introduction and increase the recruitment of general practitioners;focus on training and use,and build an interactive platform for health talent exchanges;eliminate system bottlenecks,and open up the implementation path of development of integration of prevention and treatment;change the service concept to stimulate primary health innovation and development capabilities;improve the assessment system and increase the assessment weight for the development of the integration of prevention and treatment;strengthen the linkage between the upper and lower levels,and play the role of vertical collaboration technical support;optimize the service process and strengthen informatization construction and depth of integration.These countermeasures and suggestions are aimed at strengthening the integration of prevention and treatment under the family doctor system,and providing experience and enlightenment for the development and future planning of the primary medical and health service system.
