| The first part is clinical research Objective:Observe the clinical efficacy of Shenling Baizhu Powder in treating repeated respiratory infections of children with lung-spleen-qi deficiency syndrome,and compare the TCM symptom score,the incidence of various diseases,the course of disease and the improvement of s Ig A concentration in nasal irrigation fluid before and after treatment,so as to objectively analyze the clinical effect of Shenling Baizhu powder in the treatment of the disease.Methods:The 60 children who met the inclusion criteria were randomly divided into a treatment group and a control group with 30 cases each.The treatment group was given Shenlingbaizhu powder,and the control group was given transfer factor oral liquid.After 8 weeks of treatment and 6 months of follow-up,the TCM symptom score,the incidence of various diseases,and the course of disease were observed.The nasal lavage fluid samples were collected(20 m L normal saline used in nasal lavage fluid).Measure the concentration of s Ig A in the specimen with double antibody one-step sandwich enzyme-linked immunosorbent assay(ELISA),and perform statistical analysis on the collected data.Results:(1)Symptoms of traditional Chinese medicine(main symptoms: yellow face,shortness of breath,spontaneous sweating or sweating when moving,poor appetite,abnormal stool;secondary symptoms: lack of gas,lazy speech,fear of wind): single symptom,main symptom in the treatment group scores,secondary symptoms scores,and total scores were significantly improved compared to before treatment(P<0.01);the symptoms of yellow face and lack of air and lazy talk in the control group were improved compared to before treatment(P<0.05);the other single symptoms and main symptoms were improved the scores of secondary symptoms and total scores were significantly improved compared with those before treatment(P<0.01).After treatment,the improvement of TCM symptoms in the treatment group was better than that of the control group,and it was statistically significant.The improvement of the symptoms of yellow face,shortness of breath,poor appetite,abnormal stool,and lack of gas in the treatment group was better than that of the control group(P<0.05);The improvement of spontaneous sweating,fear of wind,primary and secondary symptom scores and total scores in the treatment group were significantly better than those in the control group(P<0.01).(2)Incidence(the number and duration of the upper respiratory tract infection,tracheobronchitis and pneumonia): After treatment,the treatment group and the control group had a significant reduction in the incidence and average duration of each disease(P<0.01).There was no significant difference in the number of incidences of each disease and the average course of disease between the two groups(P>0.05).(3)The concentration of s Ig A in nasal cavity washing fluid: Compared between the two groups,the s Ig A concentration of nasal cavity washing fluid after treatment was significantly higher than that before treatment(P<0.01);there was no significant difference in s Ig A concentration in nasal cavity washing fluid after treatment(P >0.05).(4)Effective rate: The significant efficiency is 13.3%,the effective rate is 60%,and total effective rate is 73.3% in the treatment group;The significant efficiency is0%,the effective rate is 56.7%,and total effective rate is 56.7% in the control group;no effective rate after treatment between the two groups significant difference(P>0.05).Conclusion:Shenling Baizhu powder and transfer factor oral liquid can improve the incidence of recurrent respiratory tract infection in children with lung spleen deficiency type,Shenling Baizhu powder is better than transfer factor oral liquid.The treatment mechanism may be to increase the concentration of s Ig A in respiratory tract mucosa,so as to enhance the local immune ability,and then reduce the incidence of children with recurrent respiratory tract infection of lung spleen deficiency syndrome.The second part is experimental research Objective:Shenling Baizhu powder was used to intervene RRTI lung-spleen-qi deficiency model rats to observe daily weight gain,small intestine charcoal propulsion rate,thymus and spleen index,and bronchoalveolar lavage fluid(BALF)s Ig A concentration in each group to preliminarily explore their mucosal immunity Related mechanism of action.Methods:Thirty male SD rats were randomly divided into a blank control group,a model group,a western medicine group,a low-dose Chinese medicine group,a middle-dose Chinese medicine group,and a high-dose Chinese medicine group,a total of 6 groups,each with 5 rats.The model was started after 1 week of adaptive feeding.Bacterial lipopolysaccharide was dripped through the nose on the 1st and 7th days,and streptococcus pneumoniae was dripped through the nose on the 4th and 10 th days to construct a recurrent respiratory infection model;at the same time,four factors of rhubarb method,swimming exhaustion method,starvation method and smoking method were used the method of circulatory stimulation was used to construct the lung-spleen-qi deficiency model.After modeling,the blank control group was given physiological saline 1.5m L/200 g,Shenlingbaizhusan solution 1.5m L/200 g for low-dose Chinese medicine group,Shenlingbaizhusan solution 1.5m L/200 g for middle-dose group of Chinese medicine,1.5m L/200 g double-concentration of Shenlingbaizhusan for Chinese medicine high-dose group,1.5m L/200 g for high-dose group of Chinese medicine,The western medicine group was given transfer factor oral liquid 0.8m L/200 g,intragastrically,once a day for 7 days.After the dosing is over 24 hours without food and water,give carbon powder suspension(made from25 g carbon powder,5g sodium carboxymethyl cellulose,16 g milk powder and 8g white sugar in 250 m L water),2m L/200 g,irrigate After serving for 30 minutes,they were sacrificed and the intact intestine was taken out.The advance rate was calculated as the percentage of the black-stained small intestine to the total length of the intestine.The thymus and spleen were taken out and the surrounding connective tissue and fat were removed.The weight was calculated and the body mass index was calculated.The bronchus and the middle trachea were cut and inserted into the perfusion tube,and each time was filled with 2ml of pre-cooled PBS,repeated withdrawing 3 times,a total of 6ml,discarded the cell suspension,took the supernatant,and measured the s Ig A level in the specimen by(ELISA)method.Afterwards,the experimental data will be statistically analyzed.Results:(1)The daily weight gain,carbon-end small intestine propulsion rate,thymus,spleen index,and bronchoalveolar lavage fluid s Ig A levels of the model group were significantly lower than those of the normal group(P<0.01).The rats developed fever and fever after the model group was constructed.Symptoms such as coughing,loose stools,etc.,suggest that the model is successful.(2)Daily weight gain: The daily weight gain of the low-dose,medium-,and high-dose Chinese medicine group and the western medicine group all increased compared with the model group(P<0.05),and the daily weight gain of the low-dose Chinese medicine group was significantly higher than that of the model group(P<0.01).There was no statistically significant difference between the western medicine group and the low,medium and high dose groups of traditional Chinese medicine(P>0.05).(3)Carbon-end small intestine propulsion rate: the intestinal propulsion rate of the Chinese medicine middle-dose group was higher than that of the model group(P<0.05),and the intestinal propulsion rate of the Chinese medicine low-dose,high-dose group and western medicine group was significantly higher than that of the model group(P<0.01).There was no statistically significant difference between the western medicine group and the low,medium and high dose groups of traditional Chinese medicine(P>0.05).(4)Thymus index : The thymus index of the western medicine group and the low-dose Chinese medicine group was higher than that of the model group(P<0.05),and the thymus index of the middle and high-dose Chinese medicine group was significantly higher than that of the model group(P<0.01).There was no statistically significant difference between the western medicine group and the low,medium and high dose groups of traditional Chinese medicine(P>0.05).(5)Spleen index: The spleen index of the low-,medium-,and high-dose Chinese medicine group and the western medicine group was better than that of the model group(P<0.05),and the spleen index of the high-dose Chinese medicine group was significantly higher than that of the model group(P<0.01).There was no statistically significant difference between the western medicine group and the low,medium and high dose groups of traditional Chinese medicine(P>0.05)(6)The sIgA concentration of BALF specimens : The s Ig A concentration of BALF specimens in the low,medium and high dose groups of traditional Chinese medicine and the western medicine group was significantly higher than that of the model group(P<0.01).There was no statistically significant difference between the western medicine group and the low,medium and high dose groups of traditional Chinese medicine(P>0.05).Conclusion:The related mucosal immune mechanism of Shenlingbaizhu powder in the treatment of RRTI may be to improve gastrointestinal function and stimulate immune organs,so as to increase SIg A homing to the respiratory tract and play a therapeutic role. |
