Objective Stroke is a cerebrovascular illness,which possesses relatively higher mortality and body handicap rate,so it brings serious economic and social burden to patients and families after stroke.This study with the intent of exploring the correlation among plasma cystatin C and high-sensitivity C-reactive protein and the prognosis of patients with severe stroke,to provide biochemical indicators for clinical evaluation of prognosis,so as to guide the treatment of patients and formulate effective measures to improve the prognosis,so as to achieve early detection and early intervention of adverse prognosis,and ultimately reduce the serious harm to the quality of life of patients with severe stroke.Methods From January 2019 to December 2019,we have chosen many patients with severe stroke in ICU of Neurology and neurology department of the Third Clinical Medical College of Inner Mongolia Medical University according to the clinical symptoms,brain function defect level and auxiliary examination(head CT and / or MRI)and the diagnostic principles of Chinese guidelines for diagnosis and therapy of acute cerebral infarction 2018.All the patients were over 18 years old and below 80 years old.Exclusion criteria of this study include history of chronic renal insufficiency,heart disease,chronic lung disease and secondary nervous system diseases caused by infection,trauma,poisoning,tumor and blood disease.On the basis of the above criteria,add up to 120 sufferers were collected.Blood of all subjects were collected for test of plasma cystatin C and high-sensitivity C-reactive protein and other routine biochemical tests on the morning of admission,and collected basic clinical data and related information such as age,gender,smoking,drinking and hypertension were also collected at the mean time.The NIHSS scale and the modified rankin scale were used for stroke neurological assessment.The experimental data were analyzed by chi square test and t test,univariate and multivariate logistic regression analysis,Pearson correlation analysis and other statistical analysis methods.The test results with statistical significance were all P < 0.05.Results According to NIHSS score and m RS score of stroke neurological deficit scale,120 patients were divided into two groups: good prognosis group and poor prognosis group,41 of them were poor prognosis group,and the other 79 were good prognosis group.Firstly,the routine information of the two groups of sufferers,such as gender,age,weight,height,were analyzed by the corresponding statistical methods.The consequences(P > 0.05)demonstrate that there were no conspicuousness discrepancy between the two groups.After admission,NIHSS score and m RS score were used to judge the neurological deficit of cerebral apoplexy.According to the above two scores,the poor prognosis group was significantly higher than the good prognosis group(P <0.01);The content of cystatin C and high sensitive C-reactive protein in plasma was positively correlated with NIHSS score and m RS score;The single factor Logistic regression analysis displayed that the levels of TC,TG,hs-CRP and plasma cystatin C were markedly increased at admission.The statistical analysis was as follows,TC’OR with its 95% CI was 1.015(1.006~1.181);TG’OR with 95% CI was 3.857(2.573 ~6.265);High-sensitivity C-reactive protein’OR with 95% CI was 1.417(1.113~1.802);while plasma cystatin C’ was 1.457(1.152~1.843).They are suggested that TC,TG,high sensitivity C-reactive protein and plasma cystatin C are the adverse factors of short-term prognosis in patients with severe stroke.Logistic regression analysis showed that hypertension,high-sensitivity C-reactive protein and plasma cystatin C were significantly correlated with short-term prognosis of stroke(P < 0.05).Among them,high-sensitivity C-reactive protein(or: 2.021,95% CI: 1.043-3.261)and plasma cystatin C(or: 3.093,95% CI: 1.043-3.261)were significantly correlated with short-term prognosis of stroke Based on the above experimental data,plasma cystatin C and high sensitivity C-reactive protein can be used as related factors for short-term adverse prognosis of severe stroke.Conclusions Plasma cystatin C and high-sensitivity C-reactive protein are factors for short-term adverse prognosis in patients with stroke;There is a positive correlation between plasma cystatin C and high-sensitivity C-reactive protein and neurological deficits in stroke patients;The combination of plasma cystatin C and high-sensitivity C-reactive protein can be used as a clinical assessment of the prognosis of stroke patients.
