ObjectiveTumor infiltrating lymphocytes(TILs)are an important survival predictor for tumor patients,but there are few reports on the survival of TILs and SCLC patients.This study retrospectively analyzed the relationship between TILs and the prognosis of SCLC patients,in order to predict SCLC Provide reference for prognosis.Material and methodsThe clinical data of small cell lung cancer patients who visited our hospital from January 2013 to October 2019 were retrospectively collected,including gender,age,TNM stage,treatment plan,TILs,etc.SPSS 23.0 statistical software was used to analyze the factors affecting the survival time of patients.Results(1)A total of 173 patients met the inclusion criteria,159 were males and 14 were females,with an average age of 60(24-84)years,of which 83 were in the sTIL(-)group and 90 were in the sTIL(+)group.(2)As of the date of the last follow-up,the median follow-up time was 12 months(2-62),the median OS(m OS)of the entire group was 16 months,and the 1-,2-,and 3-year OS were 60.6%,30.5% and21.4% respectively.(3)Univariate and multivariate analysis showed that cM staging and thoracic radiothrapy were independent factors affecting OS.There are statistical differences in the effects of cM staging,brain metastasis and extracranial metastasis on PFS.(4)Survival analysis showed that the 1-and 3-year OS in the sTIL(-)group and sTIL(+)group were 54.8% and 71.5%;15.9% and 26.2%,P=0.029.the 1-and 3-year PFS were 27.1% and 37.1%;8.0% and 20.1%,P=0.017.The 1-and 3-year BMFS were 72.8%and 76.2%;22.3% and 59.5%,P=0.026.The OS,PFS and BMFS of the sTIL(+)group were significantly higher than those of the sTIL(-)group,and the P value was statistically significant.Multivariate COX proportional hazard model analysis showed that the sTIL(+)group was OS(P=0.028,HR 0.663 95%CI 0.459-0.957),PFS(P=0.045,HR 0.684 95%CI0.472-0.991)and BMFS(P= 0.045,HR 0.550 95%CI 0.306–0.987)independent prognostic factors.Further propensity score matching to correct the confounding bias and selection bias between the two groups showed that sTIL(+)is an independent prognostic factor affecting BMFS(P=0.030,HR 1.958 95%CI 1.066–3.599),however,there was no statistical difference between OS and PFS.ConclusioncM staging,TRT,and sTIL are independent factors that affect OS.At the same time,sTIL is an independent factor affecting PFS and BMFS in SCLC,and it can be an important indicator for predicting the prognosis of SCLC.This study is a retrospective study,and further large-sample,prospective clinical studies are still needed to verify it.
