The Risk Factors For The Shunt-dependent Hydrocephalus After Aneurismal Subarachnoid Hemorrhage

Objective: To analyze the risk factors of shunt-dependent hydrocephalus after aneurysmal subarachnoid hemorrhage and explore its prevention and treatment methods,so as to improve the clinical prognosis.Methods: A retrospective study was performed on patients diagnosed with aneurysmal subarachnoid hemorrhage who underwent surgical treatment in the Neurosurgery Department of the Second Hospital of Hebei Medical University from September 2018 to September2020.After screening,416 patients met the inclusion criteria,including 48 patients with shunt dependent hydrocephalus and 368 patients without shunt dependent hydrocephalus.The clinical characteristics of patients without shunt-dependent hydrocephalus(NO-SDHC group)and those with shunt-dependent hydrocephalus(SDHC group)were compared,and the risk factors for shunt-dependent hydrocephalus were analyzed by univariate and multivariate Logistic regression.Results: Patient age,intracerebral ventricular hemorrhage,Hunt-Hess score,Modified Fisher score,posterior circulation aneurysm,and central nervous system infection were associated with shunt-dependent hydrocephalus(P<0.05).Multiple Logistic regression analysis showed that posterior circulatory aneurysm,central nervous system infection and modified Fisher score(3-4 points)were independent risk factors for shunt-dependent hydrocephalus in patients with intracranial aneurysmal subarachnoid hemorrhage.Conclusion: Shunt dependent hydrocephalus is a common complication after aneurysmal subarachnoid hemorrhage,and posterior circulation aneurysm,central nervous system infection,and modified Fisher score(3-4)are risk factors.
