Part One Study on the relationship between EGFR gene mutation status and serum tumor marker in non-small cell lung cancerObjective: The purpose of this study was to analyze the relationship between EGFR gene mutation and serum tumor marker(STM)in non-small cell lung cancer and to explore the effect of STM on EGFR gene mutation and their predictive value.Methods: A total of 708 patients with non-small cell lung cancer who underwent pleural effusion EGFR gene detection and STM detection in our hospital from January 2015 to December 2020 were collected,and the results were statistically analyzed.Results:1.The mutation rate of EGFR gene in female and non-smoking patients was significantly higher than that in male and smoking patients(P<0.05).2.Patients with increased CEA level,increased ProGRP level,increased CYFRA21-1 level had higher EGFR mutation rate than that in normal group;SCCA normal group had higher EGFR mutation rate than that in abnormal group(P<0.05).3.ROC analysis showed that the AUC of CEA,ProGRP,CYFRA21-1and SCCA for predicting the mutation of EGFR were 0.586(P<0.001)、0.583(P<0.001)、0.545(P=0.043)、0.582(P<0.001).4.Logistic regression analysis: non-smokers(OR=0.501,P=0.009),CEA level(OR=4.388,P<0.001),ProGRP level(OR=12.564,P < 0.001),SCCA level(OR=0.300,P < 0.001)was independent influencing factors for EGFR gene mutation.Conclusions:1.EGFR gene mutations were more likely to occur in female,non-smoking patients.2.Serum CEA,ProGRP,CYFRA21-1and SCCA were correlated with EGFR mutation,which had certain value in predicting EGFR mutation status.3.Smoking history,serum CEA,ProGRP and SCCA were independent influencing factors for EGFR gene mutation.Part Two study on the relationship between EGFR gene mutation and the expression of p53,Ki67,CD44,N-cadherin and Src in non-small cell lung cancer.Objective: The expression of p53,Ki67,CD44,N-cadherin and Src were detected by pleural effusion immunocytochemistry method,and their clinicopathological features were analyzed,to explore the correlation between relevant proteins expression and EGFR mutation status.Methods: A total of 152 NSCLC patients who underwent pleural effusion EGFR gene detection in our Hospital from January 2018 to December 2020 were collected.The expressions of p53,Ki67,CD44,N-cadherin and Src were detected by cell block immunocytochemistry method,and the results were statistically analyzed.Results:1.The positive rate of CD44 in patients under 60 years old(77.55%)was higher than that in patients aged ≥ 60 years old(74.76%).The positive rate of N-cadherin in male patients(48.44%)was higher than that in female patients(25.00%)(P<0.05).2.The positive rates of CD44 and N-cadherin in the group with lymph node metastasis were 83.33% and 55.95%,which were higher than those in the group without lymph node metastasis 66.18% and 8.82%(P < 0.05).3.The results of Spearman correlation analysis showed that p53(r=0.420,P < 0.001),Ki67(r=0.420,P < 0.001)and Src(r= 0.516,P < 0.001)were positively correlated with EGFR mutation,while CD44(r=-0.102,P=0.211)and N-cadherin(r=-0.043,P=0.601)were not correlated with EGFR mutation.Conclusions:1.The positive of CD44 in patients under 60 years old and N-cadherin in male patients were higher.2.Patients with positive CD44 and N-cadherin were more likely to have lymph node metastasis3.The EGFR mutation status was positively correlated with the expression of p53,Ki67 and Src,suggesting that the tumor cells of EGFR mutation patients had stronger ability of proliferation,invasion and metastasis.
