A Study On The Relationship Among Rumination,post-traumatic Growth And Quality Of Life In Patients With Ischemic Stroke

Objectives:To understand the status of rumination,post-traumatic growth and quality of life in patients with ischemic stroke;to analyze the influencing factors of quality of life;to explore the effects of different demographic data on quality of life with ischemic stroke;to analyze the relationship among rumination,post-traumatic growth and quality of life of patients with ischemic strokeand ischemic stroke and formulating relevant intervention measures.Methods:From December in 2019 to December in 2020,convenience sampling method was used in this study to select 281 patients with ischemic stroke admitted to Shaanxi Traditional Chinese Medicine Hospital.The questionnaire tools used were general information questionnaire,Chinese Version of Event Related Rumination Inventory(ERRI-C),Chinese-Posttraumatic Growth Inventory(C-PTGI)and Quality of Life Instruments for Chronic Disease-Stroke(QLICD-ST V2.0).SPSS19.0 software was used for independent sample t-test,one-way ANOVA,Pearson correlation analysis,and multiple stepwise linear regression analysis.Results:1.The total score of rumination of patients with ischemic stroke was(30.32±7.08),the intrusive rumination dimension is(17.82±7.53),the deliberate rumination dimension was(12.82±5.11);The total score of post-traumatic growth was(51.21±15.33),the relationship with others(9.54±3.91),personal strength(8.14±2.84),self-transformation(10.45±4.19),new possibilities(9.84±4.66),and life perception(13.25±4.21);The total score of quality of life was(54.78±17.18)points,and the scores of physical function,psychological function,social function and stroke specific module were(55.02±20.40)points,(46.18±21.67)points,(45.77±18.61)points and(60.21±21.57)points respectively.2.The One-way ANOVA showed that there are statistical significance in the comparison of the total score of life quality of stroke patients with different genders,ages,educational levels,working conditions,monthly family income,understanding of the disease,type of caregivers,suffering from other chronic diseases and stroke severity in the convalescent period(P<0.05).3.Pearson correlation analysis showed that the post-traumatic growth and various dimensions of ischemic stroke patients were positively correlated with the scores of ruminant and purpose-ruminant dimensions(r_s=0.202～0.598,P_s<0.01),and negatively correlated with the scores of invasive dimension(r_s=-0.445～-0.616,P_s<0.01);The total score of life quality and each dimension were positively correlated with the scores of ruminant and purposive ruminant dimension(r_s=0.302～0.482,P_s<0.01),and negatively correlated with the scores of intrusive ruminant dimension(r_s=-0.294～-0.553,P_s<0.01);The total score of life quality and all dimensions were positively correlated with the total score of post-traumatic growth and all dimensions(r_s=0.413～0.613,P_s<0.01).4.The multiple stepwise linear regression of results showed that intrusive rumination,life perception,self-transformation,personal strength,gender,educational level,family monthly income,working situation,payment method of medical expenses,caregiver type,stroke severity explain 57.7%of variation.Conclusions:1.The ruminant meditation of patients with ischemic stroke was at a medium level,among which the scores of intrusive ruminant dimension and purpose-driven ruminant dimension were high.Therefore,it is urgent to take measures to improve their negative perceptions.2.The post-traumatic growth of patients with ischemic stroke was at a medium level,which has a large room for improvement.The highest relationship dimension scores the highes,while the life perception dimension scores the lowest.3.The quality of life of patients with ischemic stroke was at a lower middle level,and there was a lot of room for improvement.The dimension of social function has the highest score and the dimension of stroke specific module has the lowest score.4.The main influencing factors of intrusive ruminant meditation,life perception,self-transformation,personal strength,gender,educational level,monthly family income,working situation,payment method of medical expenses,caregiver type,stroke severity affecting the quality of life of patients with ischemic stroke.5.The rumination and post-traumatic growth were positively correlated with quality of life in patients with ischemic stroke,the purposeful rumination was positively correlated with post-traumatic growth and quality of life.The intrusive rumination was negatively associated with post-traumatic growth and quality of life.
