| Objective: The aim of this study is to investigate the effects of attachment and ruminant thinking on post-traumattic stress disorder and me-dication compliance in patients with stroke。Methods:1.The subjects of study:A total of 300 patients with stroke who met the clinical diagnostic criteria were selected in the inpatient department of neurology of the second hospital of Hebei medical university from December 2018 to September 2019.300 patients meeting the inclusion criteria for stroke were screened for general information Questionnaire,Relationship Questionnaire,National Institutes of Health Stroke Scale,Transient Ischemic Attack socre,Experiences in Close Relationship and Ruminative Responses Scale.The subjects were followed up 3 months later to evaluate the PCL-C questionnaire and the Morisky medication compliance scale.2.Statistical and Analysis : SPSS24 software was used for data processing.Statistical methods include general statistical description,analysis of variance,regression analysis and so on.Results: In this study,among the 274 patients,no PTSD symptoms were 29.6%,mild PTSD symptoms were 62.8%,and obvious PTSD symptoms were 7.7%.Mean score on attachment avoidance dimension was 3.18±0.85.Mean score on attachment anxiety dimension was 3.07±1.17.The mean scores on the symptom rumination dimension,the forced thinking dimension and the reflective thinking dimension were respectively 19.15±5.70,9.61±2.80,and 7.89±2.51.The differences in PTSD scores between different attachment types were statistically significant(F=5.936 P=0.006).The differences in symptom rumination dimension(F=3.712,P=0.026)and compulsive thinking dimension(F=3.909,P=0.021)of patients with different PTSD degrees were statistically significant.The differences in attachment avoidance dimension(F= 3.30 p =0.038)and attachment anxiety dimension(F= 4.185 p =0.016)in patients with different PTSD degrees were statistically significant.PTSD scores were significantly correlated with attachment anxiety(r= 0.225)and symptom rumination(r= 0.197)and forced thinking(r=0.187).Symptoms rumination and forced thinking mediated the relationship between attachment anxiety and PTSD.In each index,there was no significant difference in the degree of drug compliance,and there was no significant correlation between attachment style,ruminant thinking,PTSD score and medication adherence.Conclusions:1.Patients with stroke have a higher PTSD positive rate。2.Patients with stroke with insecure attachment and negative rumination had higher PTSD levels。3.Attachment anxiety is a positive predictor of PTSD,and symptom rumination and compulsive thinking mediate attachment anxiety and PTSD。4.Adult attachment style,ruminant thinking,and PTSD level may not predict medication adherence。... |
