Status And Influencing Factors Of Discharge Readiness Of Elderly Patients After Coronary Intervention

Objectives: In this study,we use the transition theory as the theoretical framework to understand the status quo and influencing factors of the readiness of hospital discharge of elderly patients after coronary intervention therapy,in order to actively seek intervention plans for the readiness of hospital discharge of elderly patients after coronary intervention therapy,which is of great significance to reduce unplanned readmission.Methods: This is a cross-sectional study,taking 172 elderly patients after coronary intervention from the First Affiliated Hospital of China Medical University by convenience sampling method.This study uses questionnaire survey to investigate the level of readiness of hospital discharge and its influencing factors of the elderly patients after coronary intervention,including the general information of the elderly patients after coronary intervention,Readiness of Hospital Discharge Scale,Quality of Discharge Teaching Scale,Social support scale and General Self-efficacy Scale.The collected data were statistically analyzed by SPSS22.0 software,including reliability analysis,descriptive statistics,t-test,one-way analysis of variance,Pearson correlation analysis and multiple linear regression analysis.Results: 1.The general score of readiness of hospital discharge of elderly patients after coronary intervention was(101.26±11.86).The scores of three dimensions were as follows:personal status(24.91±3.61);Adaptability(42.04±5.71);The expected support was(34.31±4.15)points.2.The total score of the quality of discharge teaching for elderly patients after coronary intervention was(144.92±14.34).The scores of three dimensions were as follows: required content(41.95±7.62);Obtained content(42.01±7.45);Teaching effect and skills(102.91±9.46).3.The total score of social support in elderly patients after coronary intervention was(40.47±5.16),and the scores of three dimensions were:objective support(10.23±1.65);Subjective support(22.46±3.16);The availability of support was 7.74±1.66.4.The total score of general self-efficacy of elderly patients after coronary intervention was(28.56±5.41).Among 172 elderly patients after PCI,there were95 patients with a moderate level of self-efficacy,accounting for 55.2% of the total number.5.Pearson correlation analysis showed that the quality of discharge teaching,social support and general self-efficacy were positively correlated with readiness of hospital discharge after coronary intervention in the elderly(P<0.05).6.T-test and one-way ANOVA results showed that gender,age,education level and marital status,type of medication and length of hospital stay had statistically significant differences in readiness of hospital discharge scores(P<0.05).7.Multiple linear regression analysis showed that general self-efficacy,quality of discharge teaching,age,social support,type of medication,education level and length of hospital stay accounted for 67.8% of the total variation in readiness of hospital discharge.Conclusion: 1.In this study,we investigated 172 elderly patients after coronary intervention.The overall readiness of hospital discharge was at a medium and high level.3.Factors influencing readiness of hospital discharge of elderly patients after coronary intervention include general self-efficacy,quality of discharge teaching,age,social support,type of medication,educational level and length of hospital stay.Among them,the more types of medication,the higher the age,the longer the hospital stay,the lower the discharge readiness of elderly patients after PCI;The higher the education level,the higher the general self-efficacy,the higher the quality of discharge guidance,and the more social support obtained,the higher the discharge readiness of the elderly patients after PCI.
