Objective:Nucleotide-binding and oligomerization domain-like receptor protein three(NLRP3)is involved in the occurence and development of early atherosclerosis, and plays an important role in the pathophysiological process of chronic inflammatory diseases.By analyzing the relationship between NLRP3 level and acute cerebral infarction, exploring the connection between serum NLRP3 level and TOAST classification and carotid atherosclerosis.In-depth exploration of the relationship between it and the severity of illness、short-term prognosis. This will have a positive meaning for the diagnosis and treatment and also help to better assess the severity and prognosis of a cerebral infarction. Methods: Selecting 114 patients who has acute cerebral infarction,75 patients who has carotid atherosclerosis,74 patients with healthy physical examination. In the case group,61 patients who did not have recurrence of infarction after 10 days of onset and were still in the hospital for review.Collecting the clinical information and blood biochemical tests of the four groups of patients, and conduct statistical analysis according to the requirements. Results: The serum NLRP3 level of the acute cerebral infarction group was higher than that of the control group and the atherosclerosis group(both P<0.001),compared with the control group, the atherosclerosis group had a higher serum NLRP3level(P=0.001).The three groups of NLRP3 levels were compared in pairs, and the results were found to be statistically significant; The NLRP3 serum levels in the epetition group were lower than the case group(P<0.001)and higher than the control group(P<0.05),and it is not statistically significant compared with the atherosclerosis group(P=0.177);Logistic regression analysis was carried out on the statistically significant indicators between the acute cerebral infarction group and the control group, and it was concluded that NLRP3 was an independent risk factor for the occurrence of the disease.(OR =2.819,P <0.001),Comparison of different etiology types among case groups showed that the serum NLRP3 level of LAA group was higher than that of SAO group(P=0.007),the serum NLRP3 level of the acute cerebral infarction group positively correlated with the degree of neurological deficit and the volume of the intracranial infarction(r=0.273,P=0.003;r=0.284,P=0.002);the extent of the decrease in serum NLRP3 levels 10 days after the onset of this disease correlated positively with the degree of recovery of the neurological deficit(r=0.249,P<0.05).Conclusions: The serum NLRP3 level of patients who is suffering from acute cerebral infarction is significantly increased, and LAA cerebral infarction is higher than that of SAO;the serum NLRP3 level can be used as an independent risk factor for the occurrence of this disease; The serum NLRP3 level can be used as an index to assess the severity of this disease. The dynamic observation of changes in serum NLRP3 levels can predict the short-term prognosis of patients with this disease.
