| Objective: To evaluate the relationship between neutrophil / lymphocyte ratio and platelet/ lymphocyte ratio on the prognosis of patients with acute cerebral infarction,providing theoretical basis for clinical diagnosis and treatment.Methods: From October 2018 to May 2019,151 consecutive patients with acute cerebral infarction admitted to the Department of Neurology,the Affilated of Shengjing Hospital of China Medical University were enrolled retrospectively.According to the modified Rankin scale(m RS)3 months after discharge from hospital,131 patients with m RS≤2 were divided into good prognosis group,20 patients with m RS>2 were divided into poor prognosis group.Gender,age,hypertension,diabetes mellitus,recurrent cerebral infarction,smoking,TOAST classification,National Institute of Health stroke scale(NIHSS)score,C-reactive protein(CRP),neutrophil / lymphocyte ratio(NLR)and platelet /lymphocyte ratio(PLR)were compared between the two groups.Logistic regression analysis was used to analyze the influencing factors of the prognosis in patients with acute cerebral infarction.The receiver operating characteristic curve(ROC)was used to evaluate the diagnostic effect of the NLR、PLR level and NLR combined with PLR on the prognosis of patients with acute cerebral infarction.Results:(1)In the poor prognosis group,the levels of recurrent cerebral infarction,acute cerebral infarction of large atherosclerosis type,NIHSS score,CRP,NLR and PLR were significantly higher than those of the good prognosis group(P<0.05);(2)The results of multivariate logistic regression analysis showed that recurrent cerebral infarction,large atherosclerotic cerebral infarction and NLR are independent risk factors affecting the prognosis of patients with acute cerebral infarction(P<0.05);(3)NLR diagnoses the prognosis of patients with acute cerebral infarction The area under the curve is 0.75,the sensitivity is 0.65,and the specificity is 0.756.The area under the curve for the diagnosis of acute cerebral infarction by PLR is 0.68,the sensitivity is 0.75,and the specificity is0.577.The area under the curve of NLR combined with PLR for predicting the poor prognosis of acute cerebral infarction is 0.75,the sensitivity is 0.5,and the specificity is0.931.There was no significant statistical difference between the three by Delong test.Conclusion: NLR,recurrent cerebral infarction,and large atherosclerotic cerebral infarction are risk factors for poor prognosis of patients.There is no significant difference in the diagnostic value of NLR,PLR and NLR combined with PLR in the poor prognosis of acute cerebral infarction. |
