Objectives:To explore the predictive effect of depression level at 8 to 20 weeks of gestation as assessed by the PHQ9 scale on gestational diabetes mellitus.Methods: Selection in October 2019 ~ 10 months in 2020 pregnant women in Shengjing Hospital,China Medical University,409,visits for the first time in at 20 weeks ago when the acquisition of the Patient’s general information,Health Questionnaire-9(Patient Health Questionnaire-9,PHQ9),regular prenatal test laboratory,24 to 28 weeks in our perfect Oral glucose tolerance test(Oral glucose how test,OGTT)clear presence of sugar metabolism disorders during pregnancy.In this study,the subjects were divided into groups according to PHQ9 scores.0-4 was divided into the first group,5-9 was divided into the second group,and≥10 was divided into the third group.SPSS 23.0 was used for statistical analysis to analyze the differences in clinical data between the three groups.The correlation coefficient between PHQ9 score and metabolic indexes was calculated by Pearson correlation analysis.Logistic regression was used to explore the correlation between PHQ9 scores before 20 weeks of gestation and the incidence of GDM.Results:1.There were significant differences in the incidence of GDM among the three groups(P < 0.01).The incidence rate of GDM in the third group was significantly higher than that in the first and the second groups.Although there was no statistical difference between the three groups in 2h postprandial blood glucose at 24-28 weeks and the area under the curve(AUC)of 60-120 points in OGTT,they still showed an increasing trend with the increase of PHQ-9 score.2.PHQ9 score is associated with fasting glucose at 24-28 weeks,postprandial 2h glucose,glycolated albumin,The area under the OGTT curve at 0-120 points(AUCGlu0-120),the area under the OGTT curve at 0-60 points(AUCGlu0-60)and the area under the OGTT curve at 60-120 points(AUCGlu60-120)were positively correlated(P < 0.05).3.After adjustment for other risk factors for GDM,it was observed that the higher PHQ9 score before8-20 weeks of gestation was associated with the greater risk of developing GDM after 24 weeks of gestation(OR=1.18,95%CI(1.09,1.27),P < 0.01).Conclusions: The depression level at 8 to 20 weeks of gestation assessed by the PHQ9 scale is a good predictor of gestational diabetes mellitus,and depression may be a potential risk factor for the development of GDM.
