| ObjectiveThis study was guided by Donabedian’s "structure-process-outcome" three-dimensional quality structure model to construct the quality evaluation index system for psychological nursing care in patients with chronic disease,and tested its reliability and validity,which provide a scientific assessment tool for psychological nursing quality management.At the same time,it also provides reference for the establishment of standard evaluation system for psychological service in China.Methods1.Literature review: The research group researched the literature databases and checked the documents or policies related to psychological nursing with chronic disease.Based on the results of the literature review,the quality evaluation index system for psychological nursing care in patients with chronic disease was initially drafted.2.Semi-structured interview: In order to deeply supplement and improve the contents of the draft of psychological nursing quality evaluation index system in patients with chronic disease,the research group conducted semi-structured interview with medical staff engaged in clinical work in related departments of chronic diseases to understand their views and suggestions on clinical psychological nursing.3.Delphi method: The Delphi method was used to select experts related to the research topic for two rounds of consultation,combined with the inclusion criteria of indicators to revise each index and then form the evaluation index system of psychological nursing quality in patients with chronic disease.4.Analytic hierarchy process: Employ Analytic Hierarchy Process and Yaahp 12.3software to calculate the weight and combination weight of each index.5.Questionnaire survey: A self-assessment questionnaire of psychological nursing quality was developed based on the constructed index system,and it was utilized to investigate 291 nurses in the chronic disease department of a Grade 3A hospital in Qingdao for testing the reliability and validity of the index system.Results1.The results of literature review: In the end,69 literatures were included in this study.According to the classification of the original authors or the understanding of the literature by the members of the research group,relevant indicators of psychological nursing quality evaluation in patients with chronic disease were extracted,and the indicators were described and classified,forming the preliminarily framework and related items of the quality evaluation index system for psychological nursing care in patients with chronic disease.2.The results of semi-structured interview: A total of 8 medical staff were interviewed,through the collation and analysis of the interview data,seven indicators were extracted,which include nursing staff allocation,knowledge and technology of nursing psychology,psychological service ability training,psychological-related regulation and norms,psychological evaluation,psychological nursing intervention,and mental health education.So as to deeply supplement and improve the contents of the preliminary draft of quality evaluation index system for psychological nursing care in patients with chronic disease.3.The results of Delphi: A total of 33 experts completed 2 rounds of expert consultations in this study,and the effective recovery rates of inquiry papers were 97.30% and 91.67%respectively;the expert’s authority coefficients were 0.819 and 0.831,which indicates that the experts were more active and authority.The coordination coefficient was W = 0.168 in the first round and W = 0.180 in the second round,and both coefficients were statistically significant(P < 0.01),indicating a high degree of consistency in expert judgment.Finally,the quality evaluation index system for psychological nursing care in patients with chronic disease was determined,which included 3 first-level indicators,13 second-level indicators and 47 thirdlevel indicators.4.The results of Analytic hierarchy process: The weights of three first-level indicators were respectively structure(0.198),process(0.491)and result(0.312).The top three of secondlevel indicators in the combination weight were knowledge,attitudes and practices of patients(0.153),psychological assessment(0.145),and psychological nursing intervention(0.145).The top five of third-level indicators in the combination weight were patient psychological nursing compliance(0.075),patients’ degree of social function recovery(0.054),awareness rate of disease(0.048),psychological problem identification rate(0.047),and follow-up service information construction(0.046).And all indicators were verified by the consistency test,and CR values are less than 0.1,which demonstrates the setting of indexes weights was reasonable.5.The results of reliability and validity of the index system: The Cronbach’s α coefficient of the total index system was 0.823,half reliability was 0.761;the item level content validity(I-CVI)of the index was 0.833～1.000,the total consistent content validity index(S-CVI/UA)was 0.810,and the average content validity index(S-CVI/Ave)was 0.968.Thirteen common factors were extracted by exploratory factor analysis,the cumulative contribution rate of variance was 76.887%,which was in accordance with the dimension of the secondary indicators of the index system.The results show that the evaluation system has high reliability and validity.ConclusionThe quality evaluation index system for psychological nursing care in patients with chronic disease had been constructed is scientific and reasonable in this study through the literature review,semi-structured interview and two-round experts’ consultations.And the reliability and validity of the index system is good,which can provide a reference for the evaluation and monitoring of the quality evaluation of psychological for patients with chronic diseases,promotes the continuous improvement of the quality of psychological nursing. |
