The Construction And Empirical Study Of The Evaluation Index System About "Combination Of Medical Care And Nursing Care" Pension Model

Objective:1.Construct the evaluation index system of "medical support and integration" mode for the aged.The framework of evaluation index system was discussed by the expert group,and the indexes were selected and revised by the Delphi expert consultation method.2.To carry out an evaluation of the work of "health care integration" in Shandong.In order to further promote and evaluate the reliability and validity of each evaluation index,the evaluation index system of medical support combined with pension model was used to monitor and evaluate the integration of medical care in Shandong,to describe the situation of the integration of medical care in Shandong,to analyze the problems and key constraints in the actual work and to test the reliability and validity of the evaluation indexes.Suggestions are provided to improve the combination of medical and medical care.Method:1.Literature review method,through consulting the domestic and foreign monographs and academic papers on medical care,fully understand the research results of the related problems related to medical care at home and abroad,and summing up the international development model,relevant policies and experience enlightenment,to provide the theory and demonstration for this study.2.Expert consultation method,uses the special panel discussion method to carry out the framework formulation and the primary selection of the index.The Delphi expert consultation method is used to construct the index system and the index selection.The results of the consultation are understood and evaluated using the authority coefficient,the coordination coefficient and the correlation coefficient,and the evaluation index system is determined and the empirical analysis is used.The data were tested for reliability and validity of the evaluation index system.3.Empirical research,the selection of shandong region to carry out empirical research,using descriptive analysis,t test,analysis of variance,correlation analysis and other methods,the description and analysis of the shandong region d combined work condition,understand the related factors.Result:1.The reliability of expert opinion: the positive coefficients of two rounds of expert consultation were 88.9% and 100% respectively,the average of expert authority coefficient was 0.80,and the coordination coefficients of one or two and three levels of second rounds of expert consultation were 0.503(P < 0.05),0.566(P < 0.05),and 0.732(P < 0.05).2.Reliability and validity: through the reliability test,the total clone of the evaluation system was 0.701(P < 0.05).Through the validity test,this study obtained 3 principal components through the principal component analysis,the total variance of the total variance was 77.242%,and the Spearman correlation coefficients between the three first indexes were 0.674,0.737 and 0.707,respectively.(P < 0.05).3.Through the empirical study,the total scores and three first grade indexes in Shandong region are(104.41±17.94),(39.71±11.39),(43.53±6.09)and(21.18±2.13),respectively.The scores of the three first grade indexes in the eastern region are respectively(36.14±14.55)points and(45.71±7.61)points.And(22.57±1.27)points;the score of the three first grade indexes in the central region is(36±6.78),(43.43±4.35)and(21.14±1.68),and the score of three first grade indexes in the western region is(33.33±3.21),(38.67±3.79)and(18.00±1.00).Conclusion:1.After two rounds of expert consultation and group discussion,the evaluation index system of "medical support combination" model was constructed from 3 first level indicators,11 two level indicators and 42 three level indicators.The experts’ degree of authority and coordination in the consultation results are within a reasonable range.2.Reliability and validity test results show that the established evaluation index system has good reliability and validity,and can be used as a tool for preliminary evaluation of the work of "combination of medical care and nourishing".3.The results of the empirical study in Shandong area show that the overall medical support work in the province is generally good,and the first grade index scores are above the standard value.The top three cities of the "medical care combination" in all cities are C City,A city and B City,and the top three cities are the city of O,J and F.Among them,the key restrictive factors that affect the work of medical and health integration in Shandong are policy implementation,system construction and social security system construction.From the region,the eastern region score were higher than in the central region and western region,the regional in policy implementation,system construction,the social security system construction,department cost score gap obvious synergy and residents.
