| Objective:Chronic dacryocystitis is a common and frequently-occurring disease in ophthalmology,and the cause is still not clear,Most clinical studies believe that nasal diseases and nasal cavity structural abnormalities are involved in the pathogenesis of chronic dacryocystitis.There is no CT examination and research on the correlation between structural abnormalities of the nasal cavity and chronic dacryocystitis.This study intends to compare and analyze the CT results of patients with chronic dacryocystitis in Qingdao Municipal Hospital and healthy physical examiners during the same period,to explore the relationship between the disease and the structural abnormalities of the nasal cavity from the perspective of imaging CT,and to demonstrate the abnormality of the nasal cavity structure It provides clinical data and imaging basis for the study of the cause of chronic dacryocystitis,and can also be used for the surgical treatment of chronic dacryocystitis,especially for nasal endoscopic dacryocystostomy and nasal correction Provide theoretical supportMethods:Fifty patients with chronic dacryocystitis admitted to the hospital from June 2017 to November 2018 were selected as the observation group;46 cases of healthy physical examination during the same period were selected as the control group.After admission to the hospital,both groups used 128-slice CT lacrimal angiography and performed three-dimensional reconstruction of the obtained data.Measure the length of the nasolacrimal duct,the upper,middle and lower meridians on the thin-layer MPR reconstructed image.Measure the length,left and right diameter,anteroposterior diameter of the lacrimal sac,observe the obstructed part of the nasolacrimal duct and the abnormal structure of the nasal cavity.Conduct statistical analysis.Results:There was a significant difference in the length of the lacrimal sac between the observation group and the control group(P<0.05).There was a statistically significant difference in the anteroposterior and left-right diameters of the lacrimal sac between the two groups(P<0.05).There was a difference in the diameter of the upper,middle and lower nasolacrimal duct in the observation group(F=10.311 P<0.001),and the difference in the diameter of the upper,middle and lower nasolacrimal duct in the control group was statistically significant(F=7.832 P<0.001).There was no significant difference between the observation group and the control group in the diameter of the upper orifice of the nasolacrimal duct(t=0.993,P=0.321).There was no significant difference in the diameter of the middle and lower orifice of the nasolacrimal duct between the two groups(t1=0.608,P1=0.544;t2=0.218,P2=0.827).There was no significant difference in the length of the nasolacrimal duct between the two groups(P>0.05).Among the 50 patients with chronic dacryocystitis in the observation group,the upper nasolacrimal duct obstruction accounted for 48.00%,the middle segment obstruction accounted for 44.00%,and the lower segment obstruction accounted for 8.00%.The obstruction was mainly the middle and upper segment;the observation group and the control group nasal cavity structure The abnormality rates were 46.00%and 21.74%,respectively.There were differences in the degree of nasal cavity structural abnormality between the two groups(χ2=10.53,P=0.001).Conclusion:The general data and CT data of the research subjects were statistically analyzed,combined with clinical operation control and the conclusions are as follows.Under CT lacrimal angiography,abnormalities in the structure of the nasolacrimal duct and nasal cavity are related to the incidence of chronic dacryocystitis,among which the deviation of the nasal septum and Mainly inferior turbinate hypertrophy.CT lacrimal duct examination can not only show the specific location of the lacrimal duct obstruction and the size of the lacrimal sac,but also determine the condition of nearby tissues and structures,which is an important help for the evaluation of the surgical plan.This study provides theoretical support for transnasal endoscopic dacryocystorhinostomy combined with nasal surgery at the same time. |
