| Objective: To evaluate the clinical efficacy,safety and recurrence of Yangxue Xiaofeng Zhiyang prescription sitz bath combined with desonide cream in the treatment of chronic perianal eczema with pattern of wind-dryness due to blood deficiency,so as to provide basis for later clinical application.Methods: The subjects of this study were all from the anorectal clinic of Henan Provincial Hospital of Traditional Chinese medicine,including 68 patients with chronic perianal eczema with pattern of wind-dryness due to blood deficiency.The patients were randomly divided into experimental group and control group,34 cases in each group.The experimental group was given sitz bath by Yangxue Xiaofeng Zhiyang prescription combined with desonide cream for external use,and the control group was given sitz bath by warm water combined with desonide cream for external use.The course of treatment was 4 weeks.The curative effect was evaluated by observing the skin lesion severity score of the two groups before and after treatment,and the dermatology life quality index(DLQI)score,eosinophils(EOS)count and adverse reactions were observed before and after treatment.After treatment,the patients were followed up for 3 months to evaluate the recurrence rate of the two groups.Results:(1)A total of 68 patients with chronic perianal eczema were included in this study.In the course of the experiment,1 case fell off in both the experimental group and the control group,and 66 cases were actually included.According to the relevant statistical analysis,there was no significant difference in gender,age and course of disease between the two groups before treatment(P > 0.05).(2)Intra-group comparison: After treatment,the lesion morphology score,symptom score,lesion severity score,DLQI score and EOS count of the two groups were significantly better than those before treatment,and the difference was statistically significant(P < 0.05).(3)Comparison between groups:(1)The effective rate of the experimental group(93.94%)was significantly better than that of the control group(78.79%),and the difference was statistically significant(P < 0.05).(2)Skin lesion morphology: the experimental group was significantly better than the control group in improving exfoliation,lichenification and skin lesion area(P <0.05),but there was no significant difference in improving erythema between the two groups(P > 0.05).(3)Conscious symptoms: the experimental group was better than the control group in improving the sense of dryness and roughness(P < 0.05),but there was no significant difference in improving the sense of pruritus between the two groups(P >0.05).(4)Severity of skin lesions: the experimental group was significantly better than the control group in improving the severity of skin lesions(P < 0.05).(5)The experimental group was significantly better than the control group in improving the quality of life and reducing the EOS count(P < 0.05).(4)There were no adverse reactions in the two groups.After 3 months of follow-up,the recurrence rate of the experimental group(6.45%)was significantly lower than that of the control group(38.46%),the difference was statistically significant(P < 0.05).Conclusions:The clinical efficacy of Yangxue Xiaofeng Zhiyang prescription sitz bath combined with desonide cream in the treatment of chronic perianal eczema with pattern of wind-dryness due to blood deficiency was significantly better than that of desonide cream alone.Yangxue Xiaofeng Zhiyang prescription sitz bath combined with desonide cream can significantly improve the clinical symptoms,improve the quality of life of patients,reduce EOS count and reduce recurrence.The results showed that Yangxue Xiaofeng Zhiyang prescription sitz bath combined with desonide cream was safe and effective in the treatment of chronic perianal eczema with pattern of wind-dryness due to blood deficiency,with low recurrence rate,which was worthy of further promotion. |
