Objectives In this study,the expression level of β-AR m RNA in peripheral blood lymphocytes of patients with heart failure was detected to explore the predictive value of β-AR m RNA expression level in patients with heart failure complicated with ventricular arrhythmia.Methods A total of 38 patients with chronic heart failure who received inpatient treatment in the heart center of Hebei Provincial People’s Hospital from April 2020 to January 2021 were selected,who were divided into chronic heart failure control group and chronic heart failure combined with ventricular arrhythmias experimental group.Patients in both groups were given standard treatment according to the current guidelines and fasting venous blood in the morning was extracted.The relative expression levels of β1-AR m RNA,β2-AR m RNA and β3-AR m RNA were detected by fluorescence quantitative PCR.The results of ventricular prephase systolic load,short ventricular tachycardia and ventricular autonomic rhythm were recorded by electrocardiogram and 12-channel dynamic electrocardiogram,that were divided into control group with simple chronic heart failure and experimental group with chronic heart failure combined with ventricular prephase contraction.Baseline data,NT-pro BNP,biochemical test results and echocardiography were compared between the two groups at admission.SPSS 26.0 was used for statistical analysis of the data,and P<0.05 was considered statistically significant.One-way ANOVA and independent sample t test were used for the measurement data subject to normal distribution,Mann-Whitney U test was used for those who did not follow the normal distribution.Chi-square test or Fisher’s exact probability method was used for counting data.The relationship between ventricular arrhythmias and β1-AR m RNA,β2-AR m RNA,β3-AR m RNA expression levels,and other indicators was analyzed by Logistic regression and receiver operating characteristic curve to evaluate the predictive value of β-AR relative expression levels in the diagnosis of patients with chronic heart failure complicated with ventricular arrhythmias.Results 1 The relative expression level of β1-AR m RNA in experimental group was significantly higher than that in control group,the relative expression level of β2-AR m RNA was not significantly different from that in control group,and the relative expression level of β3-AR m RNA was extremely significantly higher than that in control group.2 Univariate Logistic regression analysis showed that β3-AR,heart rate and triglyceride might be independent risk factors for ventricular arrhythmia(P<0.1).3Multivariate Logistic regression analysis showed that β3-AR and heart rate were independent risk factors for ventricular arrhythmias,β3-AR(OR=3.425,95%CI: 1.011-11.597,P<0.05)、heart rate(OR=0.908,95%CI: 0.830-0.993,P<0.05).4 The sensitivity and specificity of the receiver operating characteristic curve for evaluating ventricular arrhythmias showed that the area under the heart rate curve was 0.333(95%CI: 0.160-0.507,P>0.05),the area under the curve of β3-AR m RNA was 0.886(95%CI: 0.783-0.989,P<0.001).Conclusion The expression level of β3-AR is closely related to the occurrence of ventricular arrhythmias in chronic heart failure,suggesting that β3-AR is an independent risk factor for ventricular arrhythmias in chronic heart failure,and has predictive value for ventricular arrhythmias.Figure 2;Table 11;Reference 198...
