[Objective]Objective To investigate the influencing factors of Gleason score upgrading from prostate biopsy(BGS)specimens to PGS specimens after radical prostatectomy(RP).Predict clinical and pathological factors that may influence postoperative Gleason score escalation.[Methods]The preoperative and postoperative clinical and pathological examination data of 206 patients who underwent radical prostatectomy in Hangzhou First People’s Hospital from 2015 to 2018 were collected,and the factors influencing postoperative Gleason score upgrading were evaluated by Logistic regression analysis.[Results]About half of patients were identified with Gleason score escalation on final pathology,with a higher probability of escalation especially in patients with a BGS of 6.Statistical analysis showed that BMI,T-PSA,positive margin,and clinicopathological stage were independent influencing factors for postoperative GS upgrade,and had an impact on postoperative Gleason score upgrade,while age,PV,PSAD,CPPB,and prostate capsule invasion(PM)had no significant impact on postoperative GS upgrade.Not an independent risk factor for postoperative GS escalation.[Conclusion]This retrospective analysis suggested a Gleason score compliance rate of 50.5%.We identified BMI,T-PSA,positive margin and clinicopathological stage as independent risk factors for postoperative GS escalation.
