| Objective:To observe the clinical efficacy and safety of detoxification and invigorating sPleen method in the treatment of atoPic dermatitis(sPleen deficiency and toxin accumulation tyPe),and to observe the effect on the Patient’s serum total IgE level.Method:According to the inclusion criteria,a total of 60 Patients with atoPic dermatitis(sPleen deficiency and Poison accumulation tyPe)were enrolled,and 60 Patients were randomly divided into treatment grouP and control grouP,with 30 cases in each grouP.The treatment grouP was given oral administration of Jiedu JianPi Decoction,and the control grouP was given oral levocetirizine hydrochloride tablets,and both grouPs were given external 10%urea ointment;2 weeks was a course of treatment,and the treatment lasted 2 courses.Before treatment,after 2 weeks of treatment,and after 4 weeks of treatment,observe and record the Patient’ s skin lesion area score,skin lesion severity score,Pruritus(VAS)score,steeP status(VAS)score,SCORAD score,and before and after treatment.Serum total IgE value and quality of life index score,and observe its adverse reactions and the recurrence rate three months after the end of treatment,and perform statistical analysis.Results:1.The final collected cases in this study were 62 cases.One case in both the treatment group and the control group was excluded because it did not adhere to the treatment.In the end,a total of 60 Patients meeting the criteria were included.The treatment grouP and the control grouP were in gender,age,course of disease,skin lesion area before treatment,skin lesion severity,Pruritus(VAS)score,steeP status(VAS)score,SCORAD score,serum total IgE and quality of life index scores No statistical significance(all P>0.05),and comParable.2.The skin lesion area scores of the two grouPs of Patients decreased after 2 weeks and 4 weeks of treatment,and the comparison within the grouP was statistically significant(P<0.05),indicating that the two treatment methods have imProved skin lesion area;but The skin lesion area scores of the treatment grouP decreased more significantly,and the comparison between the grouPs was statistically significant(P<0.05);it shows that the addition and subtraction of Jiedu JianPi Fang is better than the control drug in imProving the skin lesion area,and the effect is more effective.fast.3.The skin lesion severity scores of the two grouPs of Patients decreased after 2 weeks and 4 weeks of treatment,and the comParison within the grouP was statistically significant(P<0.05);it shows that the two treatment methods can imProve the severity of skin lesions;However,the skin lesion severity scores in the treatment grouP decreased more significantly,and the comparisons between the grouPs were statistically significant(P<0.05);it shows that the addition and subtraction of the Jiedu JianPi Decoction is better than the control drug in imProving the severity of the skin lesions.The effect is faster.4.The Pruritus degree(VAS)scores of the two grouPs of Patients decreased after 2 weeks and 4 weeks of treatment,and the comParison within the grouP was statistically significant(P<0.05);it shows that the two treatment methods can imProve the degree of Pruritus.There was no statistically significant comparison between the two grouPs of Pruritus degree(VAS)scores(all P>0.05);it indicated that the effect of Jiedu JianPi Decoction on imProving the degree of Pruritus was not significantly different from that of the control drugs.5.After 2 weeks and 4 weeks of treatment,VAS score decreased in both grouPs,and the intra-grouP comParison was statistically significant(P<0.05);The results showed that the two treatments could imProve the steeP condition.There was no significant difference between the two grouPs(P>0.05);The results showed that there was no significant difference between the effect of Jiedu JianPi Decoction and the control drugs in imProving sleeP.6.The total score of SCORAD decreased after 2 weeks and 4 weeks of treatment in both grouPs,and the comParison within the grouP was statistically significant(P<0.05);indicating that both treatment oPtions have theraPeutic effects;but the total score of SCORAD in the treatment grouP The value decreased more significantly,and the comParison between grouPs was statistically significant(all P<0.05);it indicated that the addition and subtraction of JiedujianPifang was better than the control drug in imProving the total score of SCORAD.7.The quality of life index scores of the two grouPs of Patients decreased after treatment,and the comParison within the grouP was statistically significant(P<0.05),indicating that the two treatment oPtions can imProve the quality of life of the Patients;but the quality of life index scores of the treatment grouP decreased More,the comParison between grouPs was statistically significant(P<0.05),indicating that the addition and subtraction of Jiedu JianPi Decoction is better than the control drugs in imProving the quality of life.8.The comParison of the total serum IgE value of the two grouPs of Patients before and after treatment was statistically significant(P<0.05),indicating that the two treatment oPtions can reduce the total serum IgE value;but the comParison between the grouPs was not statistically significant(P>0.05)),indicating that the effect of Jiedu JianPi Decoction on imProving the total serum IgE value is not significantly different from that of the control drugs.9.After 2 weeks of treatment,the total effective rate of the treatment grouP(73.3%)and the control grouP(37%)was statistically significant(P=0.011<0.05);after 4 weeks of treatment,the treatment grouP(86.7%)and the control grouP(66.7%)The total effective rate is statistically significant(P=0.028<0.05);it further shows that the treatment effect of Jiedu JianPi Decoction is better than the control drugs.10.ComParison of recurrence rate:The effective,markedly effective,and cured Patients were followed uP after 3 months.The recurrence rate in the treatment grouP was 31%,and the recurrence rate in the control grouP was 65%.After Chi-square test,X2=5.339,P=0.021<0.05,has statistical significane.11.Pearson correlation analysis between the total score of SCORAD and the total serum IgE value of the two grouPs of Patients before and after treatment,the P values were both<0.05,and the r values were both>0,indicating that the two were Positively correlated.Conclusion:1.Detoxification and sPleen strengthening method for treatment of atoPic dermatitis(sPleen deficiency and toxin accumulation tyPe)has definite curative effect,low recurrence rate and high safety,esPecially in imProving the severity and area of skin lesions.2.Serum total IgE is closely related to the severity of Patients with atoPic dermatitis.Although Jiedu JianPi Decoction can reduce the serum total IgE level,it may also have other mechanisms for the treatment of atoPic dermatitis. |
