| Purpose: In this paper,the effects of two different methods on early angle closure glaucoma were studied.By comparing the changes of preoperative and postoperative intraocular pressure,anterior chamber central depth and corneal central endothelial cell density between the two groups,the effects of intraocular pressure control,anterior chamber deepening and endothelial injury were investigated.Method: Review and analysis of 100 patients with early angle-closure glaucoma diagnosed in our hospital from January 2018 to December 2019.of these,50 YAG peripheral iridotomy and 50 phacoemulsification cataract extraction combined with intraocular lens implantation were performed in laser group and superemulsion group.preoperative and postoperative 1 day,1 week,1 month after operation,intraocular pressure,anterior chamber central depth and central corneal endothelial cell density were measured respectively.Analysis of preoperative and postoperative parameter changes to compare the two treatment methods reflected in the treatment effect.Result: 1.Intra-ocular pressure:The mean preoperative intraocular pressure was(27.4±2.0)in the laser group mm Hg,and(27.5±2.1)in the superlactate group.There was no significant difference between the two groups(P=0.883,PP=0.05).d、1w、1m IOP decreased(19.7±2.1)mm Hg、(18.8±2.0)mm Hg、(18.3±2.5)after average operation in laser group and decreased to(13.5±3.1)mm Hg、(12.7±2.5)in superlast group respectively Hg、(12.1±1.9)mm Hg,the difference significant difference in intraocular pressure d、1w、1m 1 postoperative period between the two groups(P<0.05).2.Anterior chamber central depth: The average ACD of the 1d laser group increased to(2.18±0.12)mm after operation,and the average ACD of the superemulsion group increased to(2.43±0.09)mm.The average ACD of the 1w laser group increased to(2.26±0.11)mm and the average ACD of the superemulsion group increased to(2.50±0.10)mm.The average ACD of 1 m laser group increased to(2.34±0.11)mm after operation,and the average ACD of superemulsion group increased to(2.54±0.10)mm.The difference of 1 d,1 w,1 m ACD between the two groups after operation was statistically significant(p <0.05).3.Corneal endothelial cell density: The mean density of corneal endothelial cells before laser group was(2395±279)/ mm~2,superemulsion group(2467±249)/ mm~2,there was no significant difference between the two groups(P=0.176,P>0.05).The mean corneal endothelial cell density decreased to(2331±286)per mm~2 in the 1d laser group and(2231±249)per mm~2 in the superemulsion group,and there was no significant difference in between the two groups for 1d after operation(P =0.06,P >0.05).The average corneal endothelial cell density decreased to(2312±284)per mm~2 in the postoperative 1w laser group and(2140±252)per mm~2 in the superemulsion group.The average corneal endothelial cell density decreased to(2308±292)per mm~2 in the postoperative 1m laser group,and(2107±256)per mm~2 in the superemulsion group.The difference in the density of corneal endothelial cells between the two groups was statistically significant(p <0.05).Conclusion:YAG laser iris peripheral incision and cataract phacoemulsification combined with intraocular lens implantation can effectively reduce the intraocular pressure in patients with early angle closure glaucoma. |
