ObjectiveTo explore the application effect of the modified Padua scale.To verify the effect of early individualized intervention measures in reducing the incidence of lower limb venous thrombosis in patients with lung cancer.To observe the effect of clinical characteristics and pathological classification of TNM stage on the risk of venous thromboembolism in patients with lung cancer.MethodsFrom November 2016 to November 2017,392 patients with lung cancer were selected as the research object in the respiratory department of Tai’an City central hospital,and all relevant data of the patients were collected according to the subject content,among which17 cases were diagnosed as venous thromboembolism.Applied to the original Padua scale and improved version of Padua scale to evaluate 392 cases of patients,statistical analysis,the result of the score by analyzing the score and the result of the incidence of venous thromboembolism,comparison and evaluation of the two versions of Padua in table assessment for high-risk patients diagnosed with proportion of patients with venous thromboembolism,rate of comparison;The general situation of patients with lower limb venous thrombosis in lung cancer was analyzed,and the influencing factors of lower limb venous thrombosis in lung cancer patients were discussed.According to the evaluation results,individualized preventive intervention measures were adopted for the study subjects with the risk of vte,to verify the effect of the intervention measures.ResultsThe Padua scale was evaluated as 8.89 % positive rate among high-risk patients and8.42 % positive rate of the modified Padua scale.There was little difference in the measurement tables and the difference was not statistically significant(P > 0.05).The results show that both tables can be applied to clinical evaluation.The subcurve area(AUC)of the Padua scale and the modified Padua scale for assessing the risk of venous thromboembolism in patients with pulmonary malignant tumors was 0.752(95%ci =0.675-0.829)and 0.772(0.701-0.843),respectively.The sensitivity(Padua 4)and the specificity(Padua 4.5)were95%,97%,55.90% and 56.20%,respectively.The Padua scale and the modified Padua scale score had statistically significant differences in the risk assessment of venous thromboembolism in patients with lung cancer in respiratory medicine(all P < 0.05),which could be applied in clinical practice.The data of 392 patients were statistically analyzed,and it was found that 4 cases of thrombosis occurred before the diagnosis of tumor,accounting for 23.53% of the total cases of thrombosis,and 13 cases of thrombosis occurred after the diagnosis of lung cancer,accounting for 76.47% of the total cases.12 months the cumulative incidence of thrombus were 46.15%,69.23%,69.23%,76.92% and 84.62% after chemotherapy were 33.33%,50.00%,50.00%,66.67%,83.33%,14.29%,28.57% respectively after radiotherapy,42.86%,57.14%,57.14%.This shows one of lung cancer is a major risk factor for VTE occurs,as the longer the duration of lung cancer,chemotherapy and radiotherapy,the more the higher the incidence of VTE.In the study of patients with lung cancer complicated with VTE,it was concluded that smoking and drinking had little influence on the thrombosis of lower limbs.The incidence of thrombosis of lower limbs in patients with lung cancer complicated with hypertension was 35.29%,and the incidence of thrombosis of lower limbs in patients with lung cancer complicated with lung cancer was 17.67%.Adenocarcinoma accounted for 82.35% and squamous cell carcinoma 11.76%.According to the TNM stage of the patients,there were1 patient,3 patients,4 patients and 8 patients in stage I,II,III and IV respectively,and 1patients were unable to stage.Patients with later stages had a higher rate of thrombosis than those with earlier stages.In addition,the ratio of patients with VTE after prophylactic early intervention was also different and statistically significant(all P < 0.05),indicating clinical significance of prophylactic early intervention.ConclusionBoth the Padua scale and the modified Padua scale can be used for the risk assessment of venous thromboembolism in patients with lung cancer in respiratory medicine.Early clinical intervention for lung cancer patients with high risk of VTE can reduce the incidence of VTE in lung cancer patients.
