Clinical Observation Of Qinghua Kechuan Decoction In The Treatment Of Senile Chronic Bronchitis With Phlegm Heat Stagnation In The Lung

Objective: To study the efficacy and safety of Qinghua Kechuan Decoction in the treatment of elderly patients with acute attack of chronic bronchitis.Methods:according to the design of the study,62 patients with chronic bronchitis with acute attack phlegm heat depression lung syndrome were randomly divided into treatment group and control group.The treatment group was treated with the combination of Qinghua Kechuan Decoction Combined with western medicine,including anti infection and expectorant,and the control group was treated with western medicine.Before and after treatment,the TCM syndrome score,WBC,neutrophil percentage,CRP,Pa CO2,Pa CO2 and pulmonary function were observed.Finally,the statistical analysis was carried out and the efficacy of the experimental drug was evaluated.Results:(1)the baseline data analysis of the two groups before treatment: before treatment,the comparison of the two groups’ baseline data,such as sex proportion,average age,course of disease,days of acute attack,smoking history,TCM syndrome score,relevant physical and chemical indicators,was statistically significant(P>0.05),which was comparable.(2)After treatment in treatment group total effective rate was 93.3%,control group total effective rate was 86.7%,the difference between the two groups have statistical significance(P<0.05).(3)Comparison of TCM syndrome score: after treatment,the total TCM syndrome score of the two groups decreased significantly compared with that before treatment,and the difference was statistically significant(P < 0.05).After treatment,the treatment group was better than the control group in improving the total score of TCM syndrome,and the difference was statistically significant(P < 0.05).Single symptom and sign comparison: before and after treatment,cough,sputum volume,sputum color,dyspnea,chest tightness,thirst,dry stool and other symptoms of the two groups were significantly improved,the difference was statistically significant(P< 0.05).After treatment,the improvement of cough,sputum volume,sputum color,dyspnea,chest tightness,thirst,yellow urine and dry stool in the treatment group was better than that in the control group(P < 0.05).But there was no significant difference in the improvement of wheezing,lung rales and fever between the two groups after treatment(P > 0.05).Comparison of related physical and chemical indexes: the percentage of leukocytes,neutrophils,partial pressure of carbon dioxide,partial pressure of oxygen,CRP and lung function of the two groups before and after treatment were significantly improved,with statistical significance(P < 0.05).After treatment,the improvement of partial pressure of carbon dioxide,partial pressure of oxygen and lung function in the treatment group was better than that in the control group,and the difference was statistically significant.But there was no significant difference in the improvement of WBC,neutrophil percentage and CRP between the two groups after treatment(P>0.05)。Conclusion: the combination of Qinghua Kechuan Decoction and Western medicine can achieve better clinical effect than the simple western medicine treatment for the treatment of phlegm heat stagnation and lung syndrome in the acute attack of chronic bronchitis in the elderly.
