| Objective: to combine the western medicine diagnosis and classification of type 2diabetes mellitus with dyslipidemia with the objective TCM characteristic diagnosis and treatment methods of tongue diagnosis and syndrome differentiation,and put forward the diagnosis and treatment idea of disease syndrome combination and syndrome differentiation,so as to provide objective and standardized TCM syndrome differentiation basis for diabetes mellitus with dyslipidemia,so as to achieve the purpose of comprehensive prevention and treatment of traditional Chinese and Western medicine.Methods: a total of 306 patients with T2 DM who did not meet the criteria of T206 and t22020 were collected Spss25.0 was used to analyze the data.Results:1.Comparison of different lipid metabolism types:(1)comparison of baseline characteristics: there were statistical differences in BMI,height and weight.There was no difference in age,course of diabetes,smoking history and drinking history among the groups.The height,weight and BMI of the high Tg group were higher than those of the normal group,and the weight and BMI of the high Tg group were higher than those of the low HDL-C group.(2)Comparison of laboratory indexes among different lipid metabolism types: there were differences in TC,TG,LDL-C,HDL-C,FPG and Hb A1 c,but no difference in 2h PG.(3)(3)The distribution of syndrome types among different lipid metabolism types:(1)T2DM normal blood lipid group: Qi and yin deficiency syndrome;(2)high Tc,high Tg and low HDL-C groups: phlegm dampness and spleen stagnation syndrome;(3)high Tc + high Tg group: dampness heat accumulation syndrome and Phlegm Blood Stasis Syndrome;the constituent ratio of Yin deficiency and heat excess,Qi and yin deficiency and phlegm dampness and spleen stagnation syndrome was different among the groups.(4)The comparison of tongue features among different lipid metabolism groups: the characteristics of tongue texture: there was no difference in tongue color,but there was difference in ecchymosis and ecchymosis in tongue shape;there was no difference in tongue coating.(5)There was no significant difference in tongue parameters among different lipid metabolism groups,but there was significant difference in the comparison of tongue coating parameters,such as whole coating lab-l value,whole coating lab-a value,coating edge left lab-l value,coating edge left lab-a value,coating edge right lab-l value,coating edge right lab-a value and coating tip lab-a value.2.Compared with different TCM syndrome types:(1)there were statistical differences in the parameters of tongue quality except the whole tongue lab-b,left tongue lab-b,right tongue lab-b and tip of tongue lab-b.(2)Except for the lab-a value of the tip of the tongue,there were statistical differences in the parameters of the other parts of the tongue.3.The correlation between tongue parameters and general information,laboratory indicators:(1)tongue parameters in different areas have certain correlation with the course of disease,BMI,blood glucose,blood lipid indicators of T2 DM patients;(2)tongue coating parameters in different areas have certain correlation with the course of disease,BMI,blood glucose,blood lipid indicators of T2 DM patients.4.Logistic regression analysis:for T2 DM with abnormal lipid metabolism,logistic regression analysis: 2h PG is an independent risk factor for abnormal lipid metabolism.Logistic regression analysis showed that TG was the risk factor in Qi Yin deficiency syndrome,age and TC were the risk factors in phlegm dampness trapped spleen syndrome,and Hb A1 c was the risk factor in spleen kidney yang deficiency syndrome.In the syndrome of phlegm and blood stasis,the probability of syndrome type of phlegm and blood stasis is lower than other syndrome types when TC increases by one unit.Conclusion:Objective tongue features and parameters are closely related to the type of diabetes mellitus complicated with abnormal lipid metabolism,blood glucose,blood lip id and other laboratory indicators and TCM syndrome types,which can reflect the charact eristics of different pathological types and syndromes.Therefore,it is expected to become the basis of comprehensive prevention and control,curative effect evaluation index and s yndrome differentiation and treatment of diabetes mellitus complicated with abnormal lipi d metabolism. |
