Objective-To investigate the effects of blood pressure parameters,dynamic blood pressure parameters and short-term blood pressure variables in the clinic on cognitive function in different cognitive domains in patients with maintenance peritoneal dialysis complicated with hypertension.Methods-Select the second people’s hospital of changzhou and the people’s hospital of wujin maintenance peritoneal dialysis in patients with hypertension conforms to the standard of the research object,the general information collection,let patients completed the Montreal scale,to the memory,attention,executive function,language ability,visual spatial ability,abstract thinking,computing,directional forces eight cognitive function score in the field of cognitive,Clinic blood pressure and dynamic blood pressure were measured.In general data grouping,univariate analysis between groups was conducted using t test or chi-square test,and Pearson coefficient to evaluate the linear relationship between dynamic blood pressure parameters and cognitive test scores decline.Bivariate Logistic regression analysis was used to explore the influence of blood pressure parameters on cognitive test scores in various cognitive fields.Results-A total of 156 patients with maintenance peritoneal dialysis complicated with hypertension were selected as the experimental subjects,among which 96 patients with MOCA score(< 26)were assigned to group A,and 60 patients with MOCA score(≥26)were assigned to group B.There were statistically significant differences in age and blood pressure between the two groups(All P<0.05).There were no significant differences in gender,education,uric acid,creatinine,and general condition of peritoneal dialysis between the two groups(All P>0.05).1.Blood pressure parameters in the clinic had no effect on cognitive function of patients with chronic peritoneal dialysis complicated with hypertension(All P>0.05).2.Among the dynamic blood pressure parameters,the 24-hour SBP load value(OR=3.211,P =0.024),the SBP variation coefficient during the day(OR=4.291,P=0.002),and the SBP variation coefficient during the night(OR=2.181,P =0.021)were positively correlated with the decrease in attention.The coefficient of variation of systolic blood pressure during the day(OR=3.186,P =0.001),the coefficient of variation of diastolic blood pressure during the day(OR=2.712,P =0.017),the coefficient of variation of systolic blood pressure during the night(OR=2.113,P=0.020),and the coefficient of variation of diastolic blood pressure during the night(OR=2.315,P =0.020).P =0.019)was positively correlated with memory loss;24-hour systolic blood pressure load(OR=4.123,P =0.019)was positively correlated with the decline of executive function.The 24-hour SBP load(OR=4.124,P =0.028),the variation coefficient of SBP during the day(OR=3.267,P =0.038)and the variation coefficient of SBP at night(OR=2.470,P =0.039)were positively correlated with the decrease of computing power.The 24-hour SBP load value(OR=2.452,P =0.031)and daytime SBP variation coefficient(OR=3.282,P =0.017)were positively correlated with the decrease of directional force.3.Among the short-term blood pressure variables,weighted standard deviation of systolic blood pressure(OR=3.109,P=0.038)and weighted standard deviation of diastolic blood pressure(OR=2.152,P=0.030)were positively correlated with attention loss;SBP weighted standard deviation(OR=2.472,P=0.002)and diastolic blood pressure weighted standard deviation(OR=3.168,P=0.001)were positively correlated with memory decline.The weighted standard deviation of systolic blood pressure(OR=4.992,P=0.016)and diastolic blood pressure(OR=3.201,P=0.028) were positively correlated with the decrease of computing power.Conclusion-1.Blood pressure parameters in the clinic had no effect on cognitive function of patients with maintenance peritoneal dialysis complicated with hypertension.2.Some parameters of dynamic blood pressure had different effects on the decrease of attention,memory,executive function,computational ability and orientation in patients with PD complicated with hypertension,but had no effect on language ability,visuospatial ability and abstract thinking.3.Short-term blood pressure variables had different effects on the decrease of attention,memory and computational power in patients with PD complicated with hypertension,but had no effect on executive function,language ability,visual-spatial ability,abstract thinking and orientation ability.
