Objective: Through the clinical study on the application of modified Da Chaihu decoction in acute cholecystitis with gallstones,to verify the therapeutic advantages of traditional Chinese medicine in alleviating the clinical manifestations of acute cholecystitis with gallstones,and to evaluate the reliability of its clinical efficacy.In the short term,the patients can reduce pain,avoid the acute surgical treatment,make full preparation for the future elective surgery,reduce the risk and complications of surgery,and for some patients to further dissolve stone,stone treatment,prevention of stone recurrence to do a good job in the early stage,to reduce the long-term effect of repeated occurrence of acute cholecystitis.Methods: In a randomized controlled study,80 patients who met the inclusion criteria were randomly divided into the experimental group and the control group,among which 40 patients in the experimental group were treated with the combination of traditional Chinese medicine decoction and conventional western medicine,and 40 patients in the control group were treated with conventional western medicine only.Taking 7 days as a course of treatment,the remission of clinical symptoms and signs,white blood cells,CRP,Neut % index and reexamination of gallbladder ultrasonography of the 2 groups were observed,and the change indexes were recorded on the 3rd,5th and 7th day of treatment respectively.Finally,the exact efficacy of the2 groups was compared through the clinical efficacy evaluation criteria.Results: The total effective rate of the experimental group was 92.9%,and the total effective rate of the control group was 70.4%.After treatment,the two groups had different changes(P <0.05),and the curative effect of the experimental group was better than that of the control group.Treatment of 3,5 days,patients in body temperature,blood leukocyte count,neutrophil percentage and right upper quadrant pain,right upper quadrant tenderness,nausea and vomiting,stool stem node treatment effect is better than that in control group(p < 0.01 or p < 0.05),3 days trial group and control group in c-reactive protein,bitter mouth,humeral back traction pain without treatment differences(p > 0.05),5 days in 2 groups of patients with fever treatment no difference(p > 0.05),the seventh day of treatment,Experimental group in body temperature,blood leukocyte count,neutrophil percentage,c-reactive protein and TCM symptoms improve overall recovery were better than that in control group(p <0.01 or p < 0.05),abdominal ultrasound imaging,review of the experimental group is coarse,bile in the gallbladder wall perforated sexual recovery degree than those with difference changes(p < 0.05),while in the gallbladder wall recovery degree of two groups there was no significant difference(p > 0.05).Conclusion: The modified Da Chaihu decoction has good clinical value for patients with acute cholecystitis complicated with gallstones by clearing dampness-heat,activating qi and gallbladder,relieving pain combined with smoothing liver and disintegrating stones.It can significantly relieve patients’ upper abdominal pain,right upper abdominal tenderness,bitter mouth,nausea,vomiting,changes in stool and fever symptoms and signs,and has important influence on the decrease of patients’ blood regular(white blood cells,C-reactive protein,neutrophils percentage),bile sound permeability in abdominal ultrasound development and the functional recovery of gallbladder wall.It can make patients’ inflammation in the acute stage get better and fast control and make gallbladder function return to normal state.Modified Da Chaihu decoction has both short-term and long-term effects on patients with acute cholecystitis with gallstone.
