Objective Coronary atherosclerotic heart disease(CHD)is the most common type of organ disease resulting from atherosclerosis(AS).The pathogenesis of this disease is that coronary blood supply cannot meet the requirements of myocardial metabolism,dramatic temporary ischemia and hypoxia can cause the onset of angina pectoris(AP),and more serious can lead to the occurrence of acute myocardial infarction;its clinical manifestations are mainly paroxysmal anterior chest crushing pain or tightness.This study investigated the relationship between LDL-C,HDL-C,TG,TC,MPV,SUA and coronary heart disease and its coronary lesions,which is conducive to a more comprehensive understanding of the etiology of coronary heart disease and provide the basis for the screening of the population with coronary heart disease,early diagnosis of the disease and clinical manifestations in the development of the disease.Methods A hospital-based case-control study was conducted in 226 subjects admitted to A third-class hospitial from September 20,2019 to September 20,2020,including167 patients diagnosed with coronary heart disease by coronary CT and 59 healthy controls in the Department of Physical Examination.The general population characteristics of the subjects were collected,and venous blood was collected in the fasting decubitus position in the morning of the second day to examine mean platelet volume、serum uric acid、low-density lipoprotein,high-density lipoprotein, triglyceride,total cholesterol and other hematological parameters.Frequency and constituent ratio were used for statistical description.χ2 test and independent sample T test were used for statistical analysis to determine the statistical association of each factor between each group.Multiple logistic regression test was used for multivariate analysis.The test level was 0.05.Odds Ratios(OR)and 95% CI were calculated.Pearson correlation was used to analyze the association between serum uric acid levels and each index.Result1.A total of 226 subjects were included in this study,including 167 patients with coronary heart disease and 59 healthy controls.The age range of the subjects was 37-86 years old,with an average age of 59.70±9.01 years old;143(63.27%)were male and 83(36.73%)were female.In terms of gender and age,there was no statistical difference between the case group and the control group,indicating a balanced comparability in these two aspects.Smoking,low density lipoprotein,triglyceride and total cholesterol were also found to be risk factors for coronary heart disease(P < 0.05),and the mean value and proportion were higher in the case group than in the control group.However,high-density lipoprotein was a protective factor for coronary heart disease(P < 0.05),and the mean value in the case group was lower than that in the control group.2.Comparing the differences of individual general characteristics such as age,gender and alcohol consumption in Gensini low rating group,Gensini middle rating group and Gensini high rating group,it was found that there was no significant difference between the case group and the healthy control group(P > 0.05).LDL-C level was 2.86 ± 0.54 mmol / L,TG level was 1.53 ± 0.34 mmol / L,HDL-C level was 0.97 ± 0.14 mmol / L,TC level was 3.95 ± 0.33 mmol / L,MPV was10.79 ± 1.40 f L,fasting blood glucose level was 5.22 ± 1.21 mmol / L,SUA level was 313.97 ± 149.55 μ mol/L。 Through univariate analysis among the three groups,it was found that LDL-C,HDL-C,TG,TC,MPV and SUA were significantly different(P < 0.05).The results of pairwise comparison between groups showed that TG,HDL-C,TC,MPV and SUA in the middle evaluation group were significantly higher than those in the low evaluation group,while LDL-C,TG,MPV and SUA in the high evaluation group were significantly higher(P < 0.05);Compared with the middle evaluation group,LDL-C,HDL-C,TC,MPV and SUA in the high evaluation group were significantly higher(P < 0.05).3.Multivariate Logistic regression analysis was conducted with the degree of vascular disease in CHD patients as the dependent variable and the variables with statistical value as the independent variables.The group with high vascular disease in CHD patients was taken as the reference category.The analysis results showed that total cholesterol and platelet volume were the influencing factors of the degree of vascular disease in patients with coronary heart disease,with P values of 0.01 and0.00,respectively.4.Pearson correlation was used to analyze the degree of association between serum uric acid and low-density lipoprotein,triglyceride,high-density lipoprotein,total cholesterol,platelet volume and fasting blood glucose in patients with coronary heart disease.According to the results of Pearson correlation analysis,there was a positive correlation between platelet volume and serum UA level in patients with coronary heart disease,That is,with the increase of UA level in CHD patients,PLT volume also increased,platelet volume also increased,r 0.868.Conclusion1.The results of ordered multi-classification logistics regression showed that MPV and TC were the influencing factors of the degree of vascular lesions in CHD patients;2.Serum UA level was positively correlated with MPV in CHD patients;3.Smoking,LDL-C,TG,HDL-C and TC are the influencing factors of CHD patients;4.MPV,SUA,LDL-C,HDL-C,TC and TG are closely related to the degree of coronary artery lesions in CHD patients.
