Objective:This study collected the data of diagnosis and treatment of malignant tumors in Jilin city from 2017 to 2019,including the data of incidence and death of malignant tumors,analyzed the incidence and death of malignant tumors in Jilin city,monitored the distribution of malignant tumors and population,gender,age,the order of incidence and death and the changing trend,and obtained the distribution characteristics,survival time and influencing factors of incidence and death of malignant tumors in Jilin city,so as to understand the epidemic law of malignant tumors.To provide scientific basis for the government to formulate tumor control measures and evaluation of prevention and treatment effects,effectively reduce the incidence and mortality of malignant tumors,prolong the survival time of malignant tumors,and gradually improve the living quality and health level of residents.Method:The data of malignant tumor cases in this study comes from the data of tumor incidence and death reported by Jilin Cancer Registry in 2017-2019.China’s standard population uses the age composition of the sixth national census in 2010,while the world’s standard population uses Segi’s standard population composition.The population data comes from the census and the public security department in the jurisdiction.Malignant tumor cases are carried out according to the tumor section in the10 th edition of the International Classification of Diseases(ICD-10)compiled by the World Health Organization(WHO).Use SPSS 24.0 and Excel software to calculate the gross incidence(death)rate,age-standardized incidence(death)rate,gender-age incidence(death)rate,incidence(death)classification percentage and cumulative incidence(death)rate of malignant tumors.The comparison of survival of malignant tumors was analyzed by log-rank test and Cox proportional risk regression model,with the test level α=0.05.Survival and survminer packages in R software were used to draw the survival curve.Results:1.From 2017 to 2019,the total number of malignant tumors in Jilin City was27,759,with an overall incidence of 242.15/100,000,a winning bid rate of1,605.7/100,000 and a world standard rate of 122.72/100,000;Among them,the crude incidence rate of males is 245.70/100,000,that of females is 238.56/100,000,and the ratio of males to females is 1.04:1.2.From 2017 to 2019,the top five malignant tumors in Jilin were lung cancer(57.13/100,000),colorectal cancer(27.97/100,000),breast cancer(23.78/100,000),liver cancer(22.29/100,000)and gastric cancer(16.15/100,000)The top five male diseases are lung cancer(incidence rate 71.45/100,000),colorectal cancer(incidence rate 31.86%),liver cancer(incidence rate 30.93%),gastric cancer(incidence rate23.06%)and bladder cancer(incidence rate 9.64%).The top five diseases of women are breast cancer(47.43/100,000),lung cancer(39.17/100,000),colorectal cancer(24.03/100,000),cervical cancer(20.00/100,000)and thyroid cancer(15.28/100,000).3.From 2017 to 2019,the incidence of malignant tumors in Jilin City was different in different gender age groups,with males reaching the peak in 75-year-old group and females reaching the peak in 80-year-old group.The incidence of malignant tumors in different age groups is also different.The incidence of lung cancer and colorectal cancer is on the rise with age,and the incidence of those over 40 years old is obviously increased.The incidence of breast cancer is obviously increased from 25 years old,with the highest incidence in the 60-year-old group,and the incidence of liver cancer is obviously increased from 35 years old.Except for breast cancer,the incidence of other major malignant tumors is the highest in the 75-year-old group.The incidence rate of malignant tumors increased significantly from the age of 45,and the age group over 45 accounted for 91.29% of the total incidence rate of malignant tumors.The incidence rate of malignant tumors increased significantly with the increase of age.4.From 2017 to 2019,the total number of malignant tumor deaths in Jilin City was 12,485,with the overall mortality rate of 108.18/100,000,the winning bid rate of53.62/100,000 and the world standard rate of 52.53/100,000;Among them,the crude mortality rate of males is 127.14/100,000,that of females is 88.97/100,000,and the ratio of male to female mortality is 1.45:1.5.The top five causes of death in Jilin from 2017 to 2019 were lung cancer(mortality rate of 34.32/100,000),liver cancer(151.3/100,000),colorectal cancer(9.82/100,000),gastric cancer(8.56/100,000)and pancreatic cancer(mortality rate of45.7/100,000).The top five male deaths were lung cancer(42.59/100,000),liver cancer(205/100,000),stomach cancer(11.74/100,000),colorectal cancer(11.23/100,000)and pancreatic cancer(5.05/100,000).The top five female deaths are lung cancer(mortality rate 25.95/100,000),liver cancer(mortality rate 9.61/100,000),colorectal cancer(mortality rate 8.39/100,000),breast cancer(mortality rate 7.66/100,000)and gastric cancer(mortality rate 5.34/100,000).6.From 2017 to 2019,the death rate of malignant tumors in different gender age groups in Jilin City was different.The male reached the peak in the 75-year-old group and the female reached the peak in the 80-year-old group.The mortality of malignant tumors in different age groups is also different.The mortality of lung cancer has obviously increased from 45-year-old group to the peak in 80-year-old group,and the mortality of lung cancer is much higher than that of other malignant tumors.The mortality of colorectal cancer is mainly concentrated in the age group over 50,reaching the peak in 80-year-old group,and the mortality of breast cancer is low in all age groups.The mortality of malignant tumors in the age group over 45 shows an obvious upward trend.The age group over 45 accounts for a relatively high total mortality of malignant tumors,accounting for all deaths.7.By observing the survival of lung cancer,colorectal cancer,breast cancer,liver cancer and gastric cancer,there were 5,295 cases of lung cancer,3,841 cases of which survived,accounting for 73% of the observed cases,and 1,454 cases of which died,accounting for 27% of the observed cases.The average survival time was 29 months,the one-year survival rate was 75.40%,and the three-year survival rate was 33.90%.There were 2,875 cases of colorectal cancer observed,2,523 cases survived,accounting for 88% of the observed cases,and 352 cases died,accounting for 12% of the observed cases.The average survival time was 34 months,the one-year survival rate was 89.10%,and the three-year survival rate was 60.40%.2646 cases of breast cancer were observed,of which 2532 cases survived,accounting for 96% of the observed cases,and 114 cases died,accounting for 4% of the observed cases.The average survival time was 34 months,the one-year survival rate was 96.20%,and the three-year survival rate was93.20%.1894 cases of liver cancer were observed,of which 1288 cases survived,accounting for 68% of the observed cases,and 606 cases died,accounting for 32% of the observed cases.The average survival time was 27 months,the one-year survival rate was 70.60%,and the three-year survival rate was 49.70%.1492 cases of gastric cancer were observed,of which 1172 cases survived,accounting for 79% of the observed cases,and 320 cases died,accounting for 21% of the observed cases.The average survival time was 30 months,the one-year survival rate was 80.50%,and the three-year survival rate was 73.20%.8.Multivariate analysis of survival time of lung cancer,colorectal cancer,liver cancer,breast cancer and gastric cancer shows that age is an independent risk factor for the prognosis of gastric cancer patients(HR=1.277,P =0.043),sex is an independent risk factor for the prognosis of breast cancer patients(HR=3.461,P =0.034),and marital status is colorectal cancer(HR=0.550,P =0.001),the first primary tumor is an independent risk factor for the prognosis of patients with liver cancer(HR=4.393,P=0.001)and breast cancer(HR=7.300,P =0.005),and the second primary tumor is lung cancer(HR=1.452,P < 0.005)Conclusion:The incidence of malignant tumors in Jilin City in 2017-2019 was consistent with the national level,and the mortality rate was lower than the national average;The top five malignant tumors in Jilin City in 2017-2019 were lung cancer,colorectal cancer,breast cancer,liver cancer and gastric cancer,the first order of male incidence was lung cancer,the first order of female incidence was breast cancer,and the top five malignant tumor deaths were lung cancer,liver cancer,colorectal cancer,gastric cancer and pancreatic cancer,and the first order of male and female deaths was lung cancer.The incidence and mortality of malignant tumors over 45 years old in Jilin City showed a positive and significant upward trend in 2017-2019;Age is an independent risk factor for the prognosis of patients with gastric cancer,sex is an independent risk factor for the prognosis of patients with breast cancer,marital status is an independent risk factor for the prognosis of patients with colorectal cancer and liver cancer,and the order of multiple primary tumors is an independent risk factor for the prognosis of patients with lung cancer,colorectal cancer,liver cancer and breast cancer.
