Analysis Of Risk Factors For Early Recurrence Of Hepatocellular Carcinoma After Hepatectomy

Objctive:To explore the risk factors for early recurrence of hepatocellular carcinoma（HCC）after hepatectomy.Methods:A retrospective analysis was conducted for 177 patients selected by the inclusion and exclusion criteria from HCC hepatectomy Database of the Second department of Hepatobiliary and Pancreatic Surgery of the First Hospital of Jilin University between February 2020 and February2021.We divided the cohort into two groups: early recurrence group which experienced recurrence within 1 year after resection,and non-early recurrence group.Clinical data that may influence early recurrence were collected,including the characteristics of patient and liver,surgical characteristics,and tumor characteristics.The collected data was analyzed by univariate analysis and Logistic regression analysis by using SPSS22.0 software.Then,the risk factors for early recurrence of HCC after hepatectomy were concluded.Rusults:1.Early recurrence was found in 45 of 177 patients in this study,and the early recurrence rate was 25.42%.Univariate analysis showed that postoperative fasting glucose,ALP,albumin,DCP,intraoperative homologous blood transfusion,postoperative abdominal cavity effusion,the maximum tumor diameter,the tumor growth way,the intactness of tumor capsular,MVI,tumor differentiation and Ki67 were statistically significant between early recurrence group and non early recurrence group（p< 0.05）.2.Multivariate logistic regression analysis showed that ALP,DCP,intraoperative homologous blood transfusion,the maximum tumor diameter,and MVI were independent risk factors for early recurrence of HCC after hepatectomy（p< 0.05）.Conclusion:1.postoperative fasting glucose,ALP,albumin,DCP,intraoperative homologous blood transfusion,postoperative abdominal cavity effusion,the maximum tumor diameter,the tumor growth way,the intactness of tumor capsular,MVI,tumor differentiation and Ki67 were predictive factors for early recurrence of HCC after hepatectomy.2.ALP>125U/L,DCP>40ng/ml,intraoperative homologous blood transfusion,the maximum tumor diameter > 5cm,and MVI were independent risk factors for early recurrence of HCC after hepatectomy.
