| Objective:To explore the perioperative infection status of kidney transplant recipients in our hospital and analyze its influencing factors Methods:The medical records of 378 kidney transplant recipients who underwent allogeneic kidney transplantation in the First Hospital of Jilin University from January 2019 to December 2020 were retrospectively analyzed,including 78 cases of living-donor transplants from relatives and organ donation transplants after the death of citizens(organ Donation after Citizen’s Death,DD)300 cases.The recipients were divided into two groups according to the perioperative infection,namely the infected group and the control group(non-infected group).The relevant clinical data of the recipients during the perioperative period were collected through the hospital electronic medical record system and paper files.The measurement data that conformed to the normal distribution were expressed as the mean ± standard deviation((?) ± S),and the t test was used to compare between groups.Including: recipient preoperative hemoglobin and albumin;measurement data with non-normal distribution were expressed as median(M)interquartile range(IQR),and comparison between two groups was performed by Mann–Whitney U test,including:for Renal warm ischemia and cold ischemia time,recipient age,preoperative creatinine,duration of dialysis,duration of operation,duration of hospital stay,duration of catheter extraction,duration of drainage catheter extraction,and duration of stent extraction,etc.;classification data are expressed as frequency and percentage,using chisquare test to compare between groups,including: donor source,donor maintenance time,recipient gender,BMI,dialysis method,primary disease,number of transplants,diabetes,whether dialysis,postoperative perirenal hematoma,Arterial anastomosis method,left and right kidneys,kidney implant location,induction drugs,postoperative acute rejection(AR),postoperative delayed graft function(DGF),immunosuppressive regimen,antibiotics application etc.Multivariate analysis was performed using binary logistic regression analysis with dummy variables.To sum up,the risk factors of perioperative infection in kidney transplant recipients were analyzed based on the perioperative infection status of kidney transplant recipients.Results:1.The results of univariate analysis showed that: the recipient’s preoperative dialysis method,hemoglobin,operation time,postoperative perirenal hematoma,AR,DGF,hospital stay,extubation time(including urinary catheter,drainage tube,stent tube),etc.Significant influencing factors of perioperative infection in transplant recipients.2.The results of multivariate analysis showed that postoperative perirenal hematoma,AR,catheter removal time,hospital stay,and the recipient’s preoperative dialysis method were independent risk factors for perioperative infection in kidney transplant recipients,and postoperative perirenal hematoma,Postoperative AR,longer hospital stay,longer catheter removal time,and preoperative peritoneal dialysis recipients have an increased risk of perioperative infection.Conclusion:Postoperative perirenal hematoma,AR,catheter extubation time,hospital stay,and the recipient’s preoperative dialysis method were independent risk factors for perioperative infection in kidney transplant recipients. |
