| BackgroundLung cancer has the second highest incidence of all cancer types in the world and the first cancer death rate.In clinical treatment,we found that patients with early stage lung cancer had significantly better outcomes than patients with advanced stage.Therefore,the screening and diagnosis of early-stage lung cancer is a hot issue in the current clinical practice.In recent years,in-depth research on tumor immunity has provided an important reference for early screening,diagnosis and treatment of tumors.The immune system produces specific autoantibodies by recognizing tumor-associated antigens produced by tumor cells in the process of occurrence,proliferation and metastasis.These autoantibodies persist and stably exist in serum and are detected early in tumorigenesis.This makes tumor-associated autoantibody detection a potential biomarker for early screening and diagnosis of lung cancer.ObjectiveThis study aimed to evaluate the significance of seven lung cancer-related antibodies in the early diagnosis of lung cancer.MethodsA total of 142 patients who were treated in the Second Hospital of Jilin University from March 2021 to December 2021 were collected.The patients were divided into a lung cancer group(n=101)and a benign lung disease group(n=41)according to imaging and histopathology examination.A total of 60 health examiners from the medical examination center were recruited as the health control group.The levels of 7-TAAB in three groups were determined by ELISA,and the positive rates of the 7-TAAB were compared,and their sensitivity for diagnosing lung cancer,sensitivities,specificity and compared with traditional tumor markers.Explore the relationship between positive and expression levels of 7 antibodies linked to lung cancer and the clinical characteristics of lung cancer patients.Results1.The serum levels of P53、SOX2、GAGE7、GBU4-5 in the lung cancer group were higher than those in the healthy control group and the control group(P<0.05).2.The positive rates of PGP9.5、SOX2、GAGE7、GBU4-5 in the lung cancer group were highest(P<0.05).Lung cancer group(39.6%)has the higher positive rate than that in benign lung disease group(12.2%)and the healthy control group(3.3%)(P<0.05).3.The single 7-TAAB trial had a sensitivity range of 2.97%-14.85%,and the specificity range was 99.01%-100%.The AUC values of GAGE7,GBU4-5,P53,SOX2 and CAGE were 0.727,0.654,0.613,0.600 and 0.511,respectively,all of which had certain diagnostic value for lung cancer(P<0.05).The sensitivity and specificity of the combined detection of 7-TAAB were 39.6%and 93.07%,respectively,with an AUC value of 0.812.4.The sensitivity(60.00%VS 25.42%),Youden index(0.53 VS 0.18),positive predictive value(72.00%VS 68.18%),negative predictive value(75.81%VS 68.12%)and AUC value(0.844 VS 0.781)in the diagnosis of advanced lung cancer(stages III and IV)were higher than those of early stage(stage Ⅰ and Ⅱ)(P<0.05).The sensitivity of 7-TAAB in the small cell lung cancer(58.33%)was highest,and the AUC values were 0.939,0.801,and 0.790.5.The P53 serum level in the small cell lung cancer was highest(P<0.05).The level of serum MAGE A1 was higher in the " tumor diameter>3cm ",advanced lung cancer,lymph node metastasis and distant metastasis groups(P<0.05).Comparing the positive rate of 7-TAAB in patients with different clinical characteristics,the positive rate was higher in the smoking history,maximum tumor diameter>3cm,advanced lung cancer,lymph node metastasis and distant metastasis groups(P<0.05).6.Compared with 7-TAAB detection,traditional tumor markers have higher sensitivity(72.73%VS.40.91%)and lower specificity(56.25%VS.87.05%),and the combined interpretation of the two detection methods can be used for lung cancer.Diagnosis,its sensitivity and specificity were 78.79%,50.00%,AUC value of 0.826.Conclusion1.Serum detection of 7-TAAB can improve the accuracy of lung cancer diagnosis and provide some basis for early clinical lung cancer diagnosis.2.The diagnostic sensitivity of only one TAAB was low,and the combined interpretation of seven auto-antibodies had a relatively higher diagnostic performance.3.The combined judgment of serum 7-TAAB and traditional tumor marker detection results can improve the diagnostic performance of lung cancer... |
