| Objective: To understand the frailty of the elderly in Xi’an community,and analyze the influencing factors of the frailty of the elderly in the community,so as to provide clinical basis for the later intervention of the elderly in the community,provide reference for the establishment of the frailty management model,reduce the incidence of adverse events,ensure the life,health safety of the elderly in the community,and actively promote the healthy aging of the population.Methods: In this study,a cross-sectional study design was adopted.From October2020 to November 2020,using the method of two-stage cluster random sampling,taking Xi’an as a stratified factor,six communities are selected from the East,West,North and central of Xi’an,namely Daxing community,Dongguan South Street community,Chang’an Road community,Zhangjiabu community,West Street community and Taiyi Road community,and a total of 2434 elderly people were selected as the research objects.The Comprehensive Frailty Assessment Instrument(CFAI)was used to evaluate the frailty of the elderly.General data,health related data questionnaire,short-form Mini-Nutritional Assessment(MNA-SF),Pittsburgh Sleep Quality Index(PSQI),Barthel Index,8-item Morisky Medication Adherence Scale(MMAS-8),the 15-item Geriatric Depression Scale(GDS-15)and adverse events questionnaire were used to investigate the risk factors of frailty in the elderly in the community.Epidata3.0 software was used to establish the database and input data by two people,SPSS 25.0 statistical software was used to analyze the collected data.The counting data were described by frequency(n)and composition ratio(%),and the measurement data of the normal distribution were described by means±standard deviation(?±s).One-way ANOVA,independent sample t-test and multiple linear regression analysis were used to discuss the factors affecting ftility in the elderly.The test criteria were P<0.05,and the difference was statistically significant.Results: Among the 2434 subjects investigated in Xi’an community,the frailty status of the elderly is as follows: The total score of senile frailty was(40.23±8.13)points,of which physiological frailty was(6.62±1.78)points,psychological frailty was(13.35±4.04)points,social frailty was(11.97±3.91)points,environmental frailty was(8.30±3.92)points,the frailty of the elderly in community is characterized by high degree of social frailty.The results of univariate analysis showed that the elderly had different degrees of frailty in terms of gender,age,marital status,educational level,nature of previous work,lifestyle,residence style,personal monthly income,burden on families or individuals caused by medical expenses,personality,Body Mass Index(BMI),frequency of physical exercise,types of drugs taken,urinary incontinence,constipation,number of outpatient visits in the last year,number of hospitalizations in the last year,nutritional status,sleep status,ability of daily living activities,depression status,is there any fall in recent one year,is there any swallowing disorder in recent one year and is there any limb dysfunction in recent one year were statistically significant(P<0.05).The multivariate linear regression analysis results showed that showed that the effects of marital status,educational level,residence style,burden on families or individuals caused by medical expenses,frequency of physical exercise,constipation,number of outpatient visits in the last year,nutritional status,sleep status,ability of daily living activities,depression statusand is there any fall in recent one year on the total score of frailty were statistically significant(P<0.05).Conclusions: The frailty of the elderly in the community is characterized by high degree of social frailty,which should arouse the community medical staff’s attention to the social interaction and participation of the elderly in social activities.Marital status,educational level,residence style,burden on families or individuals caused by medical expenses,frequency of physical exercise,constipation,number of outpatient visits in the last year,nutritional status,sleep status,ability of daily living activities,depression status and is there any fall in recent one year are the influencing factors of the frailty of the elderly in the community.Therefore,it is necessary to add frailty screening to the routine physical examination of the elderly in the community,and take corresponding nursing intervention measures according to the above influencing factors,so as to reduce the incidence of frailty and adverse events of the elderly in the community,ensure the quality of life of the elderly. |
