Construction Of Evaluation Index System Of Nursing Quality In Infusion Port ——Based On Donabedian Quality Theory

Objective: To construct the nursing quality evaluation index system and weight of transfusion port,enrich the nursing quality evaluation index system of intravenous transfusion specialty,and provide reference for the improvement of nursing quality evaluation index system of intravenous transfusion specialty in the future.Methods: Based on Donabedian’s three-dimensional quality structure theory,the expert argumentation method is used to initially construct the nursing quality evaluation index system of the infusion port from the three aspects of structure,process and result.Using the Delphi expert correspondence method,15 experts in 4 fields were inquired by correspondence,According to expert opinions,the indicators are revised round by round,Finally,according to the importance score of the index by experts,the weight of each index is determined by the proportional distribution method,and the final evaluation index system of nursing quality of the infusion port is formed.Results:（1）The nursing quality evaluation index system of infusion port was constructed,including 3 first-level items,10 second-level items and 39 third-level items.（2）Expert reliability analysis results The rate of expert opinions is 100% and 93%,indicating that the experts are highly motivated,The authority coefficient of the two rounds of experts is 0.85,indicating that the authority of the experts is high.（3）Results of the concentration degree of the indicator system The importance scores of the first-level items fluctuate between 4.60-4.80,the importance scores of the second-level items fluctuate between 4.67-5.0,and the importance scores of the third-level items fluctuate between 4.07-5.0,indicating the degree of concentration of the indicators better.（4）Results of the degree of coordination of the indicator system After two rounds of inquiry,the overall coordination coefficient of the indicators,Kendall’s coefficient,fluctuated between 0.16 and 0.23,and the P values of the second and third-level indicators were all less than 0.05.The variation coefficients of the first-level items fluctuate between 0.07-0.13,the variation coefficients of the second-level items fluctuate between 0-0.15,and the variation coefficients of the third-level items fluctuate between 0-0.23,indicating the single item of nursing quality evaluation index system of infusion port has good coordination.（5）The weight of the indicator system In the weight of the infusion port nursing quality evaluation index system,the top three index items are the gradual reduction of the incidence of catheter-related bloodstream infection,the gradual reduction of the unplanned port withdrawal rate,and the gradual reduction of the drug extravasation rate,with a weight of 0.09.In the process indicators,the weights of the index entries are mostly 0.01,which shows that the weights of the evaluation index system tend to be indicators of the result.Conclusion: The infusion port nursing quality evaluation index system has certain scientific nature;the index system has distinct specialty characteristics,enriches the intravenous infusion specialty nursing quality evaluation index system,and has certain clinical application prospects.
