| Objective The comorbidity caused by the delayed diagnosis and treatment of bipolar disorder has attracted much attention,This study compared the serum uric acid level of patients with bipolar disorder before and after treatment and its correlation with the severity of symptoms,To verify whether higher serum uric acid levels can predict the development of mania,hypomania or mixed seizures in the future.By comparing and analyzing the changes and correlation of glucose and lipid metabolism indexes and liver function indexes in patients with bipolar disorder after taking different antipsychotics,early screening for the occurrence of comorbid metabolic disorders in bipolar disorder.Methods Experiment 1: 135 patients with bipolar disorder who were hospitalized in Anhui Mental Health Center from December 1,2019 to April 30,2021 were selected as the mania group,48 patients with bipolar disorder and depressive phase are regarded as the depression group,another 135 healthy subjects were selected as the control group.The experimental observation period was 6W,fasting elbow venous blood was collected before and after treatment to detect the level of uric acid,and evaluate patients with YMRS and HAMD,make records and perform statistical processing on all data.Experiment 2: 209 patients with bipolar disorder hospitalized in the same period were selected for grouping study,they were divided into clozapine group(group A),quetiapine group(group B),olanzapine group(Group C)and aripiprazole group(Group D),total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),fasting blood glucose(Glu),serum uric acid level(UA),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)were measured before and 6 weeks after treatment,SPSS 25.0 software was used for statistical analysis.Results Experiment 1: The serum uric acid levels and the incidence of hyperuricemia in patients in the manic group and depression group during the episode period were significantly higher than those in the control group,the difference is statistically significant(P<0.05);The serum uric acid level and the incidence of hyperuricemia in the manic group before treatment were significantly higher than those in the stable phase after treatment,and the difference was statistically significant(P<0.05);Serum uric acid levels during depressive episodes were higher than those in stable patients after treatment,and the difference was not statistically significant(P>0.05);The level of serum uric acid before and after treatment in the mania group was positively correlated with the Young’s Mania Scale,which was statistically significant(P<0.05),There was no significant statistical significance between groups before and after treatment of triglyceride levels in the four groups(P>0.05),there was no correlation between serum uric acid level and HAMD in depression group before and after treatment;The serum uric acid level and the incidence of hyperuricemia in men in the manic group were significantly higher than in women,which was statistically significant(P<0.05).Experiment 2:there was no significant change in total cholesterol level in the four groups before and after treatment(P >0.05),the level of triglyceride in group A、B and C was significantly higher than that before treatment(P<0.05),the level of triglyceride in group D was not significantly increased(P>0.05),the level of high density lipoprotein in group A decreased significantly after treatment(P<0.05),Group B、C and D did not decrease significantly(p>0.05).The level of fasting blood glucose in group A、B and D decreased after treatment(P<0.05),and the level of fasting blood glucose in group C did not decrease significantly(P>0.05).There was no significant difference in total cholesterol levels between the four groups before and after treatment(P>0.05),There was no significant difference between the four groups before treatment with HDL levels(P>0.05)and between groups after treatment(P<0.05).there were significant differences in fasting blood glucose levels between the four groups before and after treatment(P<0.05).There was no significant difference before and after treatment of serum uric acid levels in the four groups(P>0.05).The incidence of hypertriglyceridemia in groups A and C was higher(P<0.05).There was a negative correlation between triglyceride level and liver function index level in the four groups after treatment(P<0.05).Conclusion 1.The level of serum uric acid in patients with bipolar disorder is higher than that in healthy people,The level of serum uric acid in manic patients was positively correlated with the severity of symptoms,The increase of serum uric acid level may be a biomarker in patients with bipolar disorder.2.Clozapine and olanzapine have great effects on glucose and lipid metabolism and liver function in patients with bipolar disorder,There was a negative correlation between triglyceride level and liver enzyme level in patients with bipolar disorder,Patients with bipolar disorder should regularly monitor metabolic indicators to reduce the incidence of metabolic comorbidity. |
