Objective Schizophrenia is a group of serious mental diseases with unknown etiology and the main clinical manifestations of uncoordinated knowledge,emotion and intention.Cognitive impairment is usually considered as one of the core symptoms of schizophrenia.The diagnostic criteria of defective schizophrenia were preliminarily established by Kirkpatrick in 1989.Defective schizophrenia mainly evaluates six negative symptoms,such as emotional limitation,narrow emotional range and reduced interest.The corresponding literature shows that patients with defective schizophrenia may have more serious non-social cognitive defects than patients with non-defective schizophrenia.Stigma is related to self-stigmatization.Self-stigmatization is regarded as a person’s stigmatization experience for himself,including patients attributing their stigmatized beliefs,thoughts and feelings to their own psychological consequences,which may lead to the weakening of low patients’ sense of self-worth,shame,final withdrawal and isolation from society,And the sense of shame can predict the prognosis of schizophrenic patients.The sense of shame may lead to repeated hospitalization,poor medication compliance,poor recovery of social function and so on.About 1 / 3 1 / 2of schizophrenic patients have corresponding shame experience.Negative symptoms may further aggravate the patient’s sense of shame.At present,the research on stigma and cognitive function at home and abroad mostly involves the level of social cognition,but there is basically no research on the correlation between non-social cognition and stigma.This study aims to compare the differences of cognitive function and stigma between defective schizophrenic patients and non-defective schizophrenic patients with negative symptoms,the correlation between the dimensions of cognitive function and stigma was analyzed,and preliminarily explore the relationship between cognitive impairment and its negative symptoms and stigma in patients with defective schizophrenia.Methods 72 patients with stable schizophrenia were collected from Anhui Mental Health Center from November 2020 to April 2021.Their cognitive function and stigma were measured with RBANS、 stroop scale and Link series of shame scale.Then they were divided into Defective Schizophrenia(29 cases)and non-Defective Schizophrenia(43 cases)according to the Schedule for the Deficit Syndrome(SDS).40 normal controls were recruited in the surrounding community,and their cognitive function was measured by RBANS and stroop scale.SPSS 25.0 software was used for statistical analysis.For the measurement data consistent with normal distribution,two independent samples t-test was used between the two groups,one-way ANOVA was used between the three groups,and the measurement data were expressed as mean ± standard deviation.Counting data were analyzed using ?2 test.Spearman correlation analysis was used to analyze the correlation between measurement data scores of the two groups.The test level α=0.05,P < 0.05 was considered to be statistically significant.Results(1)The scores of defective schizophrenia in RBANS scale(P < 0.05)and Stigma Scale(P < 0.05)(In addition to education(P > 0.05))were significantly higher than those of non-defective schizophrenia.(2)In Stroop experiment,the experimental time of defective schizophrenics was significantly higher than that of non-defective schizophrenia(P < 0.05).(3)Spearman correlation analysis of 72 schizophrenic patients showed that the absolute value of Spearman correlation coefficient between each dimension of RBANS and devaluation discrimination,withdrawal,coping,misunderstanding,difference / shame,emotional experience and total score of shame was greater than 0.3,showing a negative correlation.The absolute value of Spearman correlation coefficient between color time and derogation discrimination / shame,emotional experience and total score of disease shame in each dimension of Stroop test is greater than 0.3,between word time and all dimensions except confidentiality,education,challenge,separation and response is greater than 0.3;(4)between color word time and all dimensions except confidentiality,education,challenge and separation is greater than 0.3,between color word time word time and stigma is greater than 0.3 except for education and separation,these absolute values greater than 0.3showed a positive correlation.(5)The correlation analysis between SDS score and stigma showed that the Spearman correlation coefficients of all dimensions except education were greater than 0.3,showing a positive correlation.Conclusion Defective schizophrenic patients showed more serious non-social cognitive impairment and more severe sense of shame than non-defective schizophrenic patients,suggesting that negative symptoms and cognitive impairment were related to the sense of shame,while correlation analysis showed that the non-social cognitive impairment of schizophrenic patients was negatively related to the sense of shame,There is a certain positive correlation between negative symptoms and stigma,which can provide corresponding reference in the relevant intervention measures for stigma in the future. |
