| Objective:To explore the correlation between different structural emphasis modes and the discovery of micro gastric lesions under white light gastroscope(WLG),so as to provide reference for improving the discovery rate of early gastric lesions under white light gastroscope(WLG).Method:2459 patients who underwent gastroscopy in the digestive endoscopy center of the first people’s Hospital of Yunnan Province from June 2021 to December 2021 were collected.In the digestive endoscopy center of our hospital,there are 4 Olympus endoscopes with the same host model(cv-290).The host parameters are set to A0,A5,B0 and B5 modes.They are grouped according to the host parameters and divided into A0,A5,B0 and B5.Endoscopy was performed by four endoscopists with endoscopy experience of about 2 years.Endoscopists were unaware of the emphasis mode and level of endoscopic structure.Patients were randomly assigned to examination tables with different emphasis modes for gastroscopy.The endoscopist shall conduct endoscopic examination and make endoscopic diagnosis.If the pathological biopsy is taken under the microscope,the comprehensive diagnosis shall be made after the pathological report.Two other gastroscope doctors who did not participate in this study rated the definition of gastroscope images.The discovery of micro gastric lesions and the image definition scores of each group under A0,A5,BO and B5 modes shall be statistically analyzed.Result:1.In this study,the discovery rates of gastric micro lesions in A0,A5,BO and B5 modes were 19.36%(109/563)、24.13%(159/659)、15.74%(91/578)、29.14%(192/659).There was no significant difference in general data,gender,age and age group among the four groups(P>0.05).2.The discovery rate and image definition score of gastric micro lesions in A5 and B5 modes were higher than those in AO and BO modes,and the difference was statistically significant(P<0.05).3.The comparison of A5 and B5 modes showed that the discovery rate of gastric micro lesions in B5 mode was higher,among which the discovery rate of erosive lesions was higher,which was statistically significant compared with A5 mode(P<0.05).There was no significant difference between A5 and B5 mode image clarity scores(P>0.05).4.Polypoid lesions were more likely to occur in the gastric body and fundus,and inflammatory lesions such as erosion were more likely to occur in the gastric antrum,the difference was statistically significant(P<0.05).5.There was no significant difference in the discovery rate of gastric micro lesions in different parts of gastric tissues under A5 and B5 modes((P>0.05)).Conclusion:1.There is a certain correlation between the pattern of gastroscopic structure emphasis and the discovery of micro gastric lesions.The discovery rate and image definition score of micro lesions are also different with different gastroscope structure emphasis modes.Under white light gastroscope(WLG),the discovery rate and image definition of micro lesions of the stomach can be improved by adjusting the gastroscope structure emphasis mode.2.The discovery rate and image definition score of gastric micro lesions in A5 and B5 modes are higher than those in A0 and B0 modes.Under WLG,selecting mode A and B with gastroscope structure emphasis level of 5 can improve the discovery rate of gastric micro lesions.3.The discovery rate of micro gastric lesions in B5 mode is higher than that in A5 mode.Therefore,it is recommended to select B5 mode for gastroscopy under WLG.4.The incidence of micro lesions varies with the location of the stomach.Inflammatory lesions tend to occur in the gastric antrum,and polypoid lesions tend to occur in the gastric body and fundus.5.In this study,the discovery rate of gastric micro lesions in different parts has no significant correlation with the selection of A5 and B5 modes.It is expected to be further studied and verified in more large sample studies,so as to better provide a basis for improving the discovery rate of gastric micro lesions under WLG in clinical work. |
