| Objective To investigate the relationship between serum magnesiumand selenium levels in the first trimester,mycoplasma and chlamydia infectio n in the genital tract before delivery,group B streptococcus(GBS)infection i n the third trimester and premature rupture of membranes(PROM)at term,a nd their clinical predictive value in the diagnosis of PROM,provide clinical e vidence for the early prevention and treatment of PROM and the need for ear ly intervention,reduce adverse pregnancy outcomes and improve neonatal surv ival.Methods Retrospective analysis from November 01,2019 to June 30,2021 in Hubei University for Nationalities Affiliated Hospital obstetrics regular obstetric examination and hospital delivery of PROM 100 cases,set as an obs ervation group,randomly selected 100 cases of normal pregnancy,no PROM as the control group.The general data of the included pregnant women,seru m magnesium and selenium levels in the first trimester,GBS in the genital tr act in the third trimester,and the test results of genital tract mycoplasma andchlamydia before delivery were collected.statistical analysis of the data was performed using SPSS 23.0 software.Using t test and chi-square test to calcu late serum magnesium,selenium content and whether there is a correlation bet ween Mycoplasma genitalium,Chlamydia,GBS infection and PROM.Single fa ctor analysis was performed on the count data,t-test was performed on the m easurement data,and logistic regression model was constructed to analyze therisk factors;Logistic regression models were constructed separately and jointl y according to serum magnesium,mycoplasma and GBS,and the receiver ope rating characteristic curve(ROC curve)was constructed from the model result s,and the area under the curve(AUC)was calculated to find the correspondi ng sensitivity and specificity,and analyze the predictive value of serum magn esium,mycoplasma and GBS in the diagnosis of PROM.According to the RO C curve,the optimal truncation value of serum magnesium was 29.0565,and all pregnant women included in the study were divided into high magnesium group and low magnesium group,and whether the difference in the incidenceof PROM,reproductive tract secretion GBS and mycoplasma infection rate b etween the two groups was statistically significant.Results 1.Compared with t he control group,the serum magnesium content,mycoplasma infection rate,c hlamydia infection rate,GBS infection rate and the number of spontaneous ab ortions in the observation group were significantly different(P=0.000<0.05,P=0.000<0.05,P=0.014<0.05,P=0.031<0.05,P=0.017<0.05),The content of selenium in the PROM group was not significantly different from that in the control gr oup(P=0.093>0.05).2.The AUC corresponding to serum magnesium,mycopl asma and GBS were: 0.638,0.780 and 0.540,the sensitivity was 54%,65%,14%,and the specificity was 72%,91%,and 94%,respectively.3.The inciden ce of PROM,reproductive tract secretion GBS and mycoplasma infection rate s in the low magnesium group were higher than those in the high magnesiumgroup,and the difference between the two groups was statistically significant(P=0.000<0.05,P=0.005<0.05,P=0.023<0.05).4.The AUC corresponding to ser um magnesium combined with GBS is 0.655,the sensitivity is 59%,and the specificity is 78%;the AUC corresponding to GBS combined with mycoplasm a is 0.815,which has a sensitivity of 75% and a specificity of 86%;the AU C corresponding to serum magnesium combined with mycoplasma is 0.836,an d its sensitivity is 78%,and the specificity is 84%;the AUC corresponding t o the combined diagnosis of serum magnesium,GBS and mycoplasma is 0.853,and its sensitivity is 87%,and the specificity is 79%.Conclusion 1.Serummagnesium content,genital mycoplasma,chlamydia and GBS infection were related to the occurrence of PROM,and the rate of mycoplasma infection,Chl amydia infection and GBS infection in pregnant women with PROM is higherthan that of normal pregnancies.No significant correlation was found betweenserum selenium content and PROM.2.Serum magnesium is the protective fac tor of PROM,mycoplasma and GBS are risk factors of PROM,and the lowe r the serum magnesium content,the higher the incidence of mycoplasma genit al tract and GBS infection,and the higher the incidence of PROM.3.Serum magnesium,GBS alone diagnosis PROM has a low predictive value,Mycopla sma alone diagnosis PROM has medium predictive value,serum magnesium,mycoplasma and GBS combined diagnosis of PROM has the highest predictiv e value. |
