| objective This subject uses the treatment of early Xingnao Kaiqiao acupuncture in patients with severe craniocerebral injury(sTBI)after craniotomy hematoma removal and step decompression,aiming to explore the effect of early acupuncture on the postoperative clinical effect of patients with severe brain injury.Methods A prospective clinical randomized controlled study was used to analyze the neurosurgery,neuro-intensive care unit(NICU)and intensive care unit(ICU)of Changsha Traditional Chinese Medicine Hospital(Changsha Eighth Hospital)admitted from May 2020 to 2020 In December,56 cases of severe head injury patients underwent craniotomy and gradual decompression surgery.Use the random number table method to register randomly according to the order of access.If the last digit of the random number is an odd number,the treatment group will receive early Xingnao Kaiqiao acupuncture treatment,and the even number or 0 will receive regular time Xingnao Kaiqiao acupuncture treatment as the control group.A total of 16 cases fell off during the treatment.Both groups of patients received conventional western medicine treatment after operation,and the relevant indicators of the two groups of patients after admission were counted,Including gender,age,cause of injury,pupils,midline deviation,hematoma volume,recovery time and recovery rate on the 7,14,21,and 30 days after surgery,GCS score on the 7,14,and 30 days after surgery,postoperative Wake up time and recovery rate on the 7,14,21,and 30 days,GOS score and ADL score(Barthel index)in January and March after surgery,S100β protein on the 1,7,14,and 21 days after surgery,And A univariate analysis was performed on each index,and then an independent sample test was performed between the two groups to compare the prognostic differences between the two groups.Results(1)General information:Comparison of gender,age,cause of injury,pupil,hematoma site,bleeding volume and degree of midline deviation,the difference is not statistically significant and comparable(P>0.05).(2)GCS score:The GCS scores of the two groups of patients on the 7,14,and 30 days after surgery were significantly higher than those before the operation,but the GCS scores of the treatment group were higher than those of the control group on the 7th,14th,and 30th day after surgery,The difference was statistically significant(P<0.05)).(3)Wake-up time:7,14,21,and 30 days after the operation,the number of patients in the treatment group was higher than that in the control group,and the recovery time after 1 month after the operation was significantly earlier than that in the control group.In the control group,the difference was statistically significant(P<0.05).(4)Awakening rate:The recovery rate of patients in the treatment group on the 7,14,21,and 30 days after surgery was higher than that in the control group,and the difference was statistically significant(P<0.05).(5)GOS score:1 month and 3 months after operation,the GOS scores of patients in the treatment group were higher than those in the control group,and the difference was statistically significant(P<0.05).(6)ADL score(Barthel index):The ADL score(Barthel index)of the treatment group was higher than that of the control groupat 1 month and 3 months after the operation,and the difference was statistically significant(P<0.05).(7)TCM syndrome score:1 month after operation,the total effective rate of TCM syndromes in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).(8)S100β protein:The concentration of S100β protein in the two groups increased on the first day after surgery,reached the highest value on the seventh day after surgery,and gradually decreased after 7 days.,and on the 7,14,and 21 days after surgery,the S100β protein concentration in the treatment group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion(1)Early acupuncture treatment of Xingnao Kaiqiao acupuncture is beneficial for patients with severe craniocerebral injury after step decompression to wake up as soon as possible,improve clinical efficacy,and improve TCM symptoms,signs and quality of life.(2)Early Xingnao Kaiqiao acupuncture therapy can reduce the level of S100β serum protein and improve the prognosis. |
