| Purpose:Through the retrospective analysis of 240 patients with chronic heart failure,this study explored the distribution of TCM syndrome types and clinical characteristics of chronic heart failure,and provided a certain reference basis for TCM syndrome differentiation and treatment.Material and method:Selected patients with heart failure who were hospitalized in the First Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from August1,2019 to December 31,2020 as the subject of retrospective study.A total of 240 eligible patients with chronic heart failure were selected according to the inclusion and exclusion criteria.Collect and analyze patient gender,age,heart failure classification,NYHA cardiac function classification,left ventricular ejection fraction,etiology,comorbid diseases,TCM syndrome diagnosis,cardiac ultrasound,laboratory test results,treatment medication and other information.The collected information is digitized,entered into Excel software,and a database is established.The data is processed and statistically analyzed using IBM SPSS26.0 software.Use the mean ± standard deviation to describe the normal data in the measurement data,and use the analysis of variance for differences between groups.Use median and quartile to describe non-normally distributed measurement data,and use rank sum test for differences between groups.The count data is expressed by rate and percentage(%),and the chi-square test is used for comparison between groups.This study analyzed the distribution of TCM syndrome types and clinical characteristics of patients with chronic heart failure,and provided a reference direction for clinical diagnosis and treatment.Results:1.Baseline characteristics: 240 heart failure patients in this survey were 32-96 years old,with an average age of 72.2±12.9 years old,of which male patients were the majority,accounting for 51.3%.The NYHA heart function classification of patients was mainly grade IV,accounting for 72.5%,and the average left ventricular ejection fraction was 48.45±11.14%.2.Etiology and comorbidities: the main cause of heart failure patients is coronary heart disease(40.8%)> hypertension(37.5%)> dilated cardiomyopathy(10.0%)> valvular heart disease(8.7%)> pulmonary heart Disease(7.5%).Complications were atrial fibrillation/atrial flutter(36.7%)> lung infection(31.2%)> type 2 diabetes(29.6%)> anemia(27.5%)>myocardial infarction(26.3%).3.Distribution of TCM syndrome types: The TCM syndrome types of patients with chronic heart failure are Qi deficiency,blood stasis and water stop syndrome(33.3%),Qi deficiency and blood stasis syndrome(28.3%),Qi and Yin deficiency syndrome(17.1%),heart and lung qi deficiency and blood Blood stasis syndrome(15.0%)and Yang Qi deficiency syndrome(5.8%).4.Classification of heart failure: Heart failure with preserved ejection fraction(HFpEF) patients are the main type of patients with heart failure,with 123(51.3%);followed by heat failure with reduced ejection fraction(HFrEF)with 72(30.42%);heart failure with mid-range ejection fraction(HFmrEF)with at least 45(18.8%).5.Clinical characteristics of HFpEF patients: the overall trend is that of female patients(53.7%),advanced age(74.34 years±12.18 years),and high systolic blood pressure(139.11±21.57 mm Hg).The cardiac ultrasound shows left ventricular diastole.And the characteristics of reduced end-systolic volume.6.Therapeutic drugs: The commonly used clinical treatment drugs for patients with chronic heart failure are diuretics(83.3%)>β-blockers(74.2%)>MRA(67.5%)> statins(60.4%)>ACEI/ARB Class(55.8%)>cardiotonic drugs(32.9%)>vasodilators(32.5%)>ARNI(5.0%).Conclusion:1.In this study,patients with chronic heart failure are mainly elderly patients,the main causes are coronary heart disease and hypertension,and HFpEF patients account for the largest proportion.2.The main clinical features of HFpEF patients are elderly,female,and more complicated with atrial fibrillation.3.The distribution of TCM syndrome types of inpatients with chronic heart failure is Qi deficiency,blood stasis and water stop syndrome> Qi deficiency and blood stasis syndrome>Qi and Yin deficiency syndrome>heart and lung Qi deficiency and blood stasis syndrome>Yang Qi deficiency syndrome,Qi deficiency,blood stasis and water stop syndrome It is the main clinical syndrome. |
