[Objective]To clarify the supportive care needs of colorectal cancer patients and its influencing factors,and to identify the population characteristics of high-need patients;based on this,to construct a supportive care WeChat public platform for colorectal cancer patients to provide effective support for patients’ self-management.[Methods]1.Research on the status of supportive care needs for colorectal cancer patients(1)From January 2020 to September 2020,a cross-sectional survey was conducted in the oncology department and radiotherapy department of four first-class hospitals in Soochow.The supportive care need was investigated by Comprehensive Needs Assessment Tool in cancer for patients;the survey tools of influencing factors included general information table,M.D.Anderson Symptom Inventory,Hospital Anxiety Depression Scale and Social Support Rating Scale.Spearman correlation analysis,univariate analysis and multiple stepwise linear regression analysis were used to explore the influencing factors of supportive care needs.The potential subgroups of supportive care needs were identified by latent class analysis;to clarify the population characteristics of high-need group,the association between the subgroups and statistical variables was analyzed by chi-square test.(2)Based on the results of quantitative research,from September 2020 to November 2020,qualitative interviews were conducted in the oncology department and radiotherapy department of two first-class hospitals in Soochow;the maximum difference method was used to recruit colorectal cancer patients,and Colaizzi analysis was used to complete the data analysis,so as to investigate in depth the supportive care needs of colorectal cancer patients.2.Construction and preliminary application of supportive care WeChat public platform for colorectal cancer patientsAccording to the results of previous investigation and literature review,the module,content presentation form and information support content of WeChat public platform were preliminarily formulated,and the information support content was revised through expert consultation,and then the construction of WeChat public platform for colorectal cancer patients was completed;a trial run of the platform was conducted in patients,and System Usability Scale was used for the usability evaluation of the platform.[Results]1.A total of 403 colorectal cancer patients were included in the cross-sectional survey,with an average age of 58.83±10.86 years,including 257 males(63.77%)and 146 females(36.23%);the average score of supportive care needs was 39.56±18.58,and the need scores of each dimension were ranked from high to low as follows:health care staff,information,hospital facilities,practical support,social/religious/spiritual support,psychological problems and physical symptoms;the need selection rate of each item was 5.96%~93.55%,and the top ten were from the "health care staff" and "information"dimension.2.Spearman correlation analysis showed that supportive care needs of colorectal cancer patients were positively correlated with symptom burden(including symptom severity and symptom distress),anxiety and depression(r=0.36~0.46,p<0.01);the need for physical symptoms,psychological problems and practical support were negatively correlated with social support(r=-0.19~-0.13,p<0.05),while the need for health care staff was positively correlated with social support(r=0.13,p<0.05).3.The results of univariate analysis showed that there were significant differences in supportive care needs of colorectal cancer patients with different gender,marital status,tumor type and treatment methods;combined with the results of correlation analysis and univariate analysis,multiple stepwise regression analysis found that anxiety,symptom distress,symptom severity and tumor type all affected supportive care needs of colorectal cancer patients,and the differences were significant,which can explain 35%of the variance.4.Latent class analysis found that there were two subgroups of supportive care needs in colorectal cancer patients,namely high-need group and low-need group:51.86%patients were classified as the high-need group and 48.14%patients the low-need group.Compared with married patients,unmarried,divorced or widowed patients had higher supportive care needs;compared with colon cancer patients,rectal cancer patients had higher supportive care needs.5.Qualitative interviews with 28 colorectal cancer patients found that their supportive care needs could be summarized as stoma specific needs,information needs,psychological needs and symptom management needs;among them,the stoma specific needs mainly include specific psychological needs and care and support needs,while the information needs mainly involve disease-related,examination and treatment,daily self-care and medical insurance reimbursement.6.The preliminary construction and application of the supportive care WeChat public platform "AI HU" for colorectal cancer patients was completed,and 248 users subscribed to the platform during the one-month trial.The average score of users’ evaluations on the usability of the platform was 75.50±10.31(the total score is 0-100,>70 is acceptable).[Conclusion]1.In the supportive care needs management of colorectal cancer patients,it is necessary to attach great importance to health care staff and information needs,focus on unmarried/divorced/widowed and rectal cancer patients,and pay great attention to reducing the symptom burden,anxiety and depression when formulating intervention strategies.2.The "AI HU" WeChat public platform constructed in this study has good usability and can be used for the management of the supportive care needs in colorectal cancer patients.
