Objective This study investigated the mental health status of pregnant women and the status of their utilization of mental health services during pregnancy.The Anderson model was used to analyze the influencing factors of the utilization of mental health services in pregnant women,and the main problems in the utilization of mental health services for pregnant women were explored.On this basis,the strategy of solving the problem is put forward,which provides a reference for the formulation and improvement of the mental health service policy for pregnant women.Methods This study combines qualitative and quantitative research,using literature research,questionnaire survey,interview,statistical analysis and so on.By using the method of literature research,the existing literature research and policy documents are reviewed comprehensively to form the theoretical basis of this study.Based on Anderson model,a questionnaire survey was conducted among 570 pregnant women and medical staff in Baise City,Guangxi.In order to improve the research content and understand the actual situation,interviews were conducted with pregnant women in clinics and relevant staff of local medical institutions.The collected data were analyzed and sorted out.The data were input into Excel database.The data were analyzed by SPSS22.0 statistical software,including descriptive analysis and univariate analysis(Chi-square test),multivariate analysis(Logistic regression).Results 1.General situation: There were 570 respondents,the average age was(30.7 + 5.3)years,182 Han(31.9%)and 347 Zhuang(60.9%).A total of257 cases(45.1%)of primary pregnant women and 303 cases(54.9%)of pregnant women who has borne a child.There were 38 cases(6.7%)of early pregnancy,212 cases(37.2%)of middle pregnancy and 320 cases(56.1%)of late pregnancy.153(26.8%)of them had received education from college for professional training,followed by 145(25.4%)of them received education from senior high school or polytechnic school.The main occupation is unemployment,accounting for 20.7%,followed by service industry or workers15.1%,individual workers 14.7%,teachers or technicians 13.5%,agricultural workers 12.5%,medical workers 9.1%,government workers 8.2%,and other industries 5.8%.2.Mental health status of pregnant women: the positive rate of depression screening in pregnant women was as high as 56.7%;the positive rate of anxiety screening was 34.7%.Among them,mild depression accounted for 49.3%,moderate depression accounted for 6.7%,and moderate to severe depression accounted for 0.7%.Mild anxiety accounted for 28.2%,moderate anxiety accounted for 6.0%,and severe anxiety accounted for 0.5%.There was a statistically significant difference in the degree of depression among pregnant women in terms of educational level,ethnicity,family monthly income,and family care.There is a statistically significant difference in The anxiety status of pregnant women between occupation and family care also has statistically significant difference.3.Development of mental health services for pregnant women: Pregnant women in Baise municipal medical institutions responded more to mental health services for pregnant women than in county-level medical institutions.The main contents of mental health services for pregnant women in medical institutions are mental health propaganda and promotion,and mental health lectures.The main forms of mental health services are newspaper publicity and pregnant women’s schools.Obstetricians are the main staffs of mental health service for pregnant women in Baise medical institutions,followed by obstetric nurses.Most of the medical staff have not been trained in mental health knowledge.The county maternal and child health hospital has the highest untrained rate,which reaches 85.7%.Mental health education is the main content of training for medical staff in municipal and county medical institutions.The proportion of respondents is 86.4% and 100% respectively.4.The utilization of mental health services for pregnant women: Among the respondents,some forms of mental health services received by formal medical institutions accounted for 38.1%.In the case of psychological distress,the number of respondents who requested help from their husbands was the highest(66.4%).Others were themselves(59.7%),friends(55.7%)and parents(52.7%),and 28.1% sought help from obstetricians.Asking a psychologist for help is 3.2%.Pregnant women who actively seek formal medical institutions for psychological assistance account for only 23.9%.5.Influencing factors of health service utilization: Using Anderson’s behavior model of health service utilization as the analysis framework,the influencing factors of pregnant women’s mental health service utilization were analyzed comprehensively and systematically.(1)Univariate analysis showed that the main factors affecting the utilization of mental health services for pregnant women included the residence of pregnant women,average monthly family income,type of medical insurance,pregnancy status,family care,life satisfaction,mental health service awareness,attitude of doctors,privacy protection,time for waiting,skills of doctors,high cost and environment of clinics.(2)Multivariate analysis showed that the age of pregnant women,cognition of mental health service in residence,long waiting time,doctors’ professional skills and family care were included in the regression equation.Discussion Based on the empirical research,it is concluded that pregnant women’s psychological problems are prevalent and the utilization level of mental health services during pregnancy is not high.In view of the current utilization of pregnant women’s mental health services,the propaganda of pregnant women’s mental health services is not wide enough,and pregnant women’s awareness of mental health services is low.Pregnant women’s mental health service security system is not perfect,the content and form of mental health service provided by medical institutions are inadequate,the accessibility of mental health service professionals during pregnancy is not high,and the ability and level of mental health service provided by medical institutions are insufficient.It is suggested that the government should improve the guarantee policy of mental health services for pregnant women,increase the policy guarantee and financial support for mental health services during pregnancy,improve the ability and level of mental health services in medical institutions during pregnancy,strengthen mental health education during pregnancy and improve the mental health beliefs of pregnant women.
