| Objective: 1.To search for indicators that can be used to predict the presence of MVI in HCC preoperatively.2.To evaluate the value of tumor size and eosinophils in predicting whether HCC has MVI.Methods: A total of 134 patients with HCC who received liver tumor resection from January 1,2018 to June 30,2019 in the First Affiliated Hospital of Guangxi Medical University.and their pathological findings were reported as hepatocellular carcinoma.According to the postoperative pathological examination results,there were divided into two groups:the experimental group with MVI:(45 cases)and the control group without MVI(89 cases).Univariate and binary Logistic regression were used for multivariate analysis to identify independent risk factors for the occurrence of MVI,and preoperative tumor maximum diameter and eosinophilic granulophils were preliminarily established to predict the diagnostic model of MVI.The ROC curve could be used to evaluate the diagnostic effectiveness of the model.The area under the curve(AUC),sensitivity and specificity were obtained.Finally,Hosmer-Lemeshow goodness of fit test was performed to analyze the calibration degree of the diagnostic model,and P<0.05 was considered statistically significant.Results:(1)Univariate analysis showed that Platelet(P=0.038),eosinophil(P=0.018)、AST(P=0.015)、tumor size(P=0.002)、DCP(P=0.015)AFP(P=0.015)、 China liver cancer staging(CNLC)(P<0.001)were significantly different between the two groups(P<0.05)while age,gender,HBs Ag,hepatitis B DNA,Neutrophils,lymphocytes,PLR,NLR,ALT,Albumin,total protein,pre-albumin,total bilirubin,tumor pathological grading were no different.(2)Multivariate Logistic regression analysis: showed that tumor size(P=0.026)、eosinophil(P=0.017)both P<0.05 which proved that they were independent risk factors for MVI of HCC(3)The predictive diagnosis model of eosinophil combined with tumor size for MVI was established..The results of ROC curve analysis showed when using tumor size to predicting MVI in HCC,the AUC was0.670,the sensitivity was 55.3% and the specificity was 79.8%,The AUC,sensitivity and specificity of eosinophil in predicting MVI in HCC were 0.677,73.3%and 57.3%.The AUC,sensitivity and specificity of tumor size combined with eosinophil in predicting MVI in HCC were 0.677,73.3%and 57.3%Conclusion :(1)Tumor size and eosinophil were independent factor influencing the occurrence of MVI in HCC.(2)tumor size combined with eosinophil has good predictive value of MVI in HCC.(3)Platelet,AST,AFP and DCP were not independent factor influencing the occurrence of MVI in HCC. |
