Objective: By analyzing the imaging characteristics of X-ray calcification of ductal carcinoma in situ(DCIS),to explore its relationship with clinical,histopathology and molecular biology,and then to evaluate the biological behavior of DCIS from the perspective of imaging,and guide the selection of clinical treatment to a certain extent.Methods: A retrospective analysis was performed on 182 female patients with DCIS confirmed by surgery and pathology in our hospital from January2017 to January 2021,and collects breast X-ray photography and pathological data.The expression of ER、PR、Her-2、Ki-67 and histological grade in tissues of patients was detected by immunohistochemical method.According to the result of mammography results,they were divided into calcified group and non-calcified group,and non-high-grade group and high-grade group according to histological grade.Analyzing the correlation between mammography and pathological features of DCIS.The continuous data tested using the independent sample t test or the Mann-Whitney U test.The categorical data tested using the chi-square test or Fisher exact test.Inspection levelα=0.05,P value<0.05 was considered statistically significant.Results:(1)ER negative(P=0.002),PR negative(P<0.001),Her-2 positive(P=0.035),high Ki-67(P=0.013),high nuclear grade(P<0.001),comedo necrosis(P<0.001)and mastectomy(P=0.002)were more common in calcified DCIS,compared with non-calcified DCIS,the differences were statistically significant.(2)Calcification morphology was correlated with histological grade of DCIS(P=0.016)and Her-2 expression(P=0.002).Fine linear or fine linear branching calcification(32.5%)was the most common in high-grade DCIS,and fine linear or fine linear branching calcification(41.2%)was also the most common Her-2 positive;There was no correlation between the morphology of calcification and the expression of ER,PR and Ki-67(all P>0.05).(3)The distribution of calcification was correlated with Her-2 expression(P=0.002),and Her-2 positive was the most common calcification group distribution(31.4%);There was no correlation between the distribution of calcification and the histological grade of DCIS,the expression of ER,PR and Ki-67(all P>0.05).(4)There was no correlation between mammary density,the number of calcifications and the histological grade of DCIS,the expression of ER,PR,HER-2 and Ki-67(all P>0.05).(5)ER negative(P<0.001),PR negative(P<0.001),Her-2 positive(P<0.001)and high Ki-67(P=0.026)were more common in comedo necrosis(all P<0.05),compared with non-comedo necrosis,the difference was statistically significant.Conclusion: There is a correlation between calcification on mammography and pathological features of DCIS,which can predict histological grade and molecular biomarker expression,preliminarily evaluate the invasive potential of DCIS,to provide reference for making an individualized treatment plan.
