| Objective: To observe the clinical efficacy of the “Wu Yin Tiao Shen” therapy on the symptoms of depression and secondary insomnia in post-stroke depression(PSD)patients,and to investigate the correlation between depression,insomnia,quality of life and fatigue in PSD patients.METHODS: 60 PSD patients with insomnia symptoms who were hospitalized or outpatients of the Rehabilitation Hospital of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine were randomly divided into “ Wu Yin Tiao Shen ” group and repetitive transcranial magnetic stimulation(r TMS)group by random number table method,30 patients in each group,one case was shed in each of “Wu Yin Tiao Shen” group and r TMS group.Both groups were treated with unified clinical conventional therapy for stroke,The “Wu Yin Tiao Shen” group uses acupuncture of Bai Hui,Yin Tang,and Shen Ting combined with angular tuning five elements music therapy.In the r TMS group,patients were treated with r TMS at a frequency of 1 time/day and 5times/week for 4 weeks.The clinical efficacy was evaluated before and after4 weeks of treatment using the Pittsburgh sleep quality index(PSQI),the Hamilton depression scale(HAMD-17),the Stroke-specific quality of life scale(SS-QOL),and the Fatigue Scale-14(FS-14).The PSQI and HAMD-17 subtraction rates were used to compare the clinical efficacy,and the data were statistically analyzed.RESULTS: There was no statistically significant difference between the baseline data,pre-intervention PSQI,HAMD-17,SS-QOL,and FS-14 scores of the two groups(P > 0.05).After the intervention,the PSQI,HAMD-17,SS-QOL,and FS-14 scores of patients in both groups improved significantly compared with those before the intervention,and the differences were statistically significant(P < 0.05).After the intervention,the differences between the PSQI,HAMD-17,and SS-QOL scores of the two groups were not statistically significant(P > 0.05),and the FS-14 score improvement in the “Wu Yin Tiao Shen” group was better than that in the r TMS group(P < 0.05).After the intervention,the effective rate of insomnia was 93.10% and depression was93.10% in the r TMS group,and the effective rate of insomnia was 89.66% and depression was 93.10% in the “Wu Yin Tiao Shen” group.The differences in clinical efficacy of insomnia and depression were not statistically significant(P > 0.05).There was a positive linear correlation between changes in PSQI scores and changes in HAMD-17 scores and FS-14 scores,and a negative linear correlation with changes in SS-QOL scores.CONCLUSIONS: 1.The “Wu Yin Tiao Shen” therapy can effectively improve depression,secondary insomnia and fatigue in post-stroke depression patients,and improve the ability of daily life activities,and compared with the r TMS group,the efficacy of depression,insomnia and quality of life improvement was comparable,but the effect of relieving fatigue was better.It is suggested that the treatment of PSD and secondary insomnia by the “Wu Yin Tiao Shen”therapy has good efficacy and is worthy of clinical promotion.2.The improvement of sleep quality was directly linear with the improvement of depression,quality of life and fatigue,and the improvement of sleep quality was conducive to the relief of depression,fatigue and quality of life. |
